**Background**

Ontario Health Teams (OHTs) are designed to improve the delivery of integrated and patient-oriented care. At maturity, OHTs will provide seamless, coordinated services across the health care continuum, including acute care, primary care, home care, and community services.

The Brantford Brant Norfolk Ontario Health Team (BBNOHT) was approved by the Ministry of Health in 2020 and has since advanced population health strategies in priority areas such as addictions and mental health, dementia care, homelessness, and digital health.

As the BBNOHT evolves, the Director, Digital Health and Decision Support will play a key role in advancing digital health strategies that intersect across the OHT, the Primary Care Network (PCN), and Interprofessional Primary Care Team (IPCT) expansion. This role will ensure digital tools and decision support systems enable high-quality, coordinated, and equitable care delivery.

**Position Summary**

Reporting to the Director/Executive Lead, BBNOHT the Director, Digital Health and Decision Support provides overall leadership for the design, implementation, and evaluation of digital health and decision support strategies. The Director will work in direct partnership and collaboration with the GRCHC Director of Quality and Primary Care Integration, lead a growing digital health team and foster system-wide collaboration across primary care, community health, hospital, Indigenous, and other health and social services partners.

Key leadership priorities include:

- Advancing BBNOHT and PCN digital health initiatives in alignment with provincial policy and local priorities.
- Embedding digital health as an enabling service for care transformation and integration.
- Shaping the future digital and data strategy for the OHT, with an emphasis on innovation and interoperability.
- Developing and supporting a high-performing digital health team to deliver on strategic goals.
- Fostering partnerships and collaboration with stakeholders including Ontario Health, the Ministry of Health, Indigenous partners, primary care, and community organizations.

**Primary Responsibilities**
- Provide strategic leadership to design, implement, and evaluate the BBNOHT digital health and decision support strategy, ensuring alignment with OHT, PCN, and IPCT goals.
- Oversee development and implementation of digital health projects, including virtual care, information-sharing platforms, clinical decision support, and population health analytics.
- Lead the integration of digital health solutions into clinical pathways and models of care, ensuring they support patient-centered, equitable, and culturally safe care.
- Establish governance and accountability structures for digital health initiatives, including clear performance metrics and reporting.
- Build and lead a digital health team, providing mentorship, professional development, and performance oversight.
- Collaborate with internal and external stakeholders to align digital health strategies, share best practices, and support system-level innovation.
- Manage vendor relationships, funding allocations, and business cases for digital health investments.
- Ensure digital health initiatives reflect equity, diversity, inclusion, and anti-racism principles, with meaningful engagement of Indigenous and marginalized communities.
- Oversee risk management, privacy, and security considerations for digital health and decision support initiatives.
- Champion change management efforts to build digital and data capacity across the system.

**Skills and Experience**
- Master’s degree in Health Administration, Health Informatics, Business Administration, or related field (or equivalent experience).
- Minimum 5 years of progressive leadership experience in digital health, health informatics, or health system transformation.
- Demonstrated success in leading strategy development and implementation at a system or organizational level.
- Proven experience managing multi-stakeholder digital health projects in a healthcare setting.
- Strong understanding of Ontario’s health system, including OHTs, primary care, and community health.
- Knowledge of digital health technologies, population health management, and clinical decision support tools.
- Expertise in change management, stakeholder engagement, and team development.
- Excellent analytical, communication, and problem-solving skills.
- Strong commitment to equity, diversity, inclusion, anti-racism, and Indigenous reconciliation.

**Work Environment**:

- This position offers a hybrid work arrangement, combining both remote and in-office work. At this time, the in-office expectation is 3 days per week at the B
