What You’ll Be Doing:

- Participate and/or lead funding initiatives related to medication safety and technological enhancements to continually enhance quality care
- Participate and/or lead analysis of sentinel and adverse events
- Work collaboratively with pharmacy vendor and communities to implement medication programs aligning with provincial requirements
- Engage with IPAC Partners and physicians/nurse practitioners to implement and sustain antibiotic stewardship program(s).
- Provide education to team members, residents and families on various health-related medication safety topics as required, including group presentations internally and externally
- Contribute to the development of medication management policies and procedures
- Conduct quality audits to determine effectiveness of medication services provided and assist in setting and resourcing directions for change;
Education & Requirements:

- Registered as a Pharmacist with the Ontario College of Pharmacists
- Registered with the College of Pharmacists of British Columbia, as asset
- Awareness of national pharmacy evidence based practices and best practices guidelines
- Proven experience as clinical pharmacist working in seniors care
- Experience in adult learning principles
- Effective verbal and written English communication skills required.
- Evidence of ongoing professional development and a demonstrated commitment to continuous learning
- Highly motivated; demonstrates an ability to be self-directed, working independently and as a member of a collaborative team
- Good organization and self-management skills
- _Ability to travel, at times extensively; dependent upon location, job may be remote or hybrid_

- You must be legally eligible to work in Canada or have a valid work permit
- You must successfully pass a Criminal Records Check & provide 2 references
