New Dawn Medical Walk-In Clinic is focused on primary health care with utmost attention and professionalism. We are a Walk In Clinic alongside a Family Practice, with many sister clinics across Ontario. We are currently seeking Primary Health Care Providers for our clinic in Oshawa ON area to join our team with a potential to travel when needed.
The Primary Health Care Provider reports directly to the Clinic Medical Director and is also accountable for meeting the primary term care policies, procedures, and protocols for medical services. The Primary Health Care Provider is an autonomous primary care provider to patients and works within her/his legislative scope of practice as described and outlined by the College of Physicians and Surgeons of Ontario. The Primary Health Care Provider collaborates with the staff and, and the health care team in the development, implementation and evaluation of the Walk-in plan of care. The Primary Health Care Provider must have valid CPSO and billing number registration and First aid CPR/BLS certificate.
Responsibilities include but are not limited to:
- Provides comprehensive primary care to patients as part of the interdisciplinary health care team.
- Conducts person-centered comprehensive health histories.
- Engages patient s in dialogue to determine what is important to them for health and quality of life.
- Provides person-focused health education.
- Manages the care of patients by providing pharmacological, complementary and/or counselling interventions, and performs procedures within the Primary Health Care Provider scope of practice.
- Orders and/or performs appropriate screening and diagnostic investigations, interpreting results, and assuming responsibility for follow-up.
- Diagnose acute and chronic health conditions.
- Collaborates and consults with physicians, the Medical Director of Nursing, nursing staff, interdisciplinary team members and external resources regarding a patient's plan of care.
- Makes referrals to specialized consultants, services, and other health providers.
- Documents clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, patient responses, and clinical rationale in a timely and accurate manner.

We are looking for someone to start ASAP.
Wage: Based on Fee For Services claimed to The Ministry of Health and 3rd Party on a monthly basis
(ie. $24,000 - $50,000 per month)
