NB Health Link (NBHL) leverages technological solutions and innovative clinical delivery models to provide access to care for New Brunswickers who don't have a family doctor or nurse practitioner. The program provides patients who are registered with the program access to a network of family doctors and nurse practitioners throughout the province - offering in-person, telephone and online appointments in both official languages.

**JOB SUMMARY**
Reporting to the manager, the Team Leader is part of the NB Health Link team. The Team Leader is a crucial team member who, with the support from the clinical manager, provides leadership and direction to the clinical team. The Team Leader will also fulfill a Care Coordinator role, depending on assignment.
**KEY RESPONSIBILITIES**
The Team Leader will perform a variety of duties, those include but are not limited to:
- Assuring the leadership and supervision of the clinical staff and is also the point of contact for them.
- Troubleshooting the day-to-day obstacles in clinic.
- Planning the clinical staff schedule and overseeing the changes as needed while having a high regard for optimizing patient’s schedules between providers.
- Coordinating with the multidisciplinary team and the leadership team to offer optimal care to the patients.
- Acting as liaison with the management team for the overall report of clinical efficiency and challenges.
- Acting as an ambassador for the program by promoting the program’s values and mission.
- Ensure patient records contain all relevant medical information.
- Demonstrate compliance with all program’s specific performance metrics, participate in the evaluation and/or measurement of these metrics, and plays an active role in improving personal, team and program performance.
- Uses expert knowledge and professional judgment to assess the patient’s chief complaint/concern and provides approved interventions based on caller needs.
- Review care path recommendation with the patient that is congruent with evidence base practice.
- Builds credible and therapeutic relationship with the patient by communicating effectively, articulating information clearly, and using appropriate language, clarifying the message to the receiver, validating understanding, and demonstrating cultural competence.
- Process calls and online inquires based on NBHL’s Policies and Procedures.
- Adheres to processes to provide service in a care effective and time efficient manner for all patients.
- Identify and assess appropriate patients for alternate services and communicate information to service provider.
- Maintain effective working relationships with physicians and nurse practitioners, hospital unit managers and staff, community agencies and partners.
- Promote and communicate the care plan goals to the NBHL Interdisciplinary Health Care Team.
- Lead and participate in patient care conferences to help coordinate services and share information related to known and potential NBHL patients.
- Coordinate care planning with patients, caregivers, and healthcare team, including physicians and nurse practitioners, to facilitate Primary care.
- Such other functions as may be assigned by the Director or Manager, which are consistent with the role of the Triage Nurse.
- Assist in orientation/education of new physicians/nurse practitioners/community partners and new staff regarding the NBHL program.
- Other responsibilities assigned by the medical director or manager.
**REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS**:
- Registered in good standing with the Nurses Association of New Brunswick
- BCLS certification and maintain competence.
- Three (3) years’ experience in acute care setting. Other nursing lead specialty clinic experience may be considered.
- Demonstrated commitment in achieving goals and objectives by using transformational leadership skills in all interactions.
