**REGISTERED PRACTICAL NURSE (25-R196)** **Bargaining Unit: OPSEU** **Status: Full Time** **Department: High Support Housing** **Location: SJ Health Centre** **Shifts: Days including weekends** **Rate of Pay: $34.86 - $35.30** per hour** **Positions Available: 2** Mental Health High Support Housing serves clients with serious, persistent and stable mental illness living in various homes throughout the community and follows the Psychosocial Rehabilitation Model of Care. 24-hour clinical support and observation is provided in the homes in collaboration with other disciplines to service and meet the unique needs of clients that require a higher level of support. Nursing provides ongoing wellness planning, including treatment, rehabilitation and recovery to achieve and sustain clients optimal level of mental health, wellbeing and quality of life. **Qualifications**: - Currently registered with the College of Nurses of Ontario - Scope of Practice must include physical assessment and pharmacology - Current Basic Cardiac Life Support and First Aid Certificates - Experience working with people with a serious mental illness and an intellectual disability and/or concurrent addiction issues - Extensive knowledge, skills and experience in community assessment, psychosocial rehabilitation and treatment, crisis management and recovery - Mental health experience required.
