Your Opportunity:
The Health Care Aide (HCA) provides patient centred care and will be a key member of the collaborative team to deliver quality health care. Reporting to the Clinic Manager, the HCA is responsible for maintaining a hygienic, well equipped, and safe patient care environment for the Ambulatory Medicine Clinics. The incumbent must be able to recognize, prevent and respond to potential safety risks including emergency situations; demonstrate organizational, time-management, strong communication and problem-solving skills; participate in quality control process for equipment and supplies; assist with respiratory care, response, and monitoring; measurement of vital signs; and assist with simple wound care and dressing changes. Other duties include but not limited to the following: hazardous waste management; transporting of lab samples, supplies, and equipment; and transferring patients of varying sizes and degrees of mobility. The HCA is accountable for the activities assigned and is responsible to ask the health care professionals for assistance as appropriate. The HCA will accurately document patient observations and actions in the patient’s health record and demonstrate the principles of privacy and confidentiality.

Description:
As a Health Care Aide (HCA), you will work as a collaborative member of the multi-disciplinary team providing personal assistance including supporting activities of daily living and support services to patients (clients, residents) who require short-term assistance or ongoing support, in accordance with the patient care plan and facility policies and procedures. In this position you will work closely with patients, families, and caregivers spanning the continuum of care in home, community or health care facilities assisting nurses with the provision of routine care to meet patients' hygiene, nutrition, mobility, recreation and safety needs, assisting with organized activities or programs and observing and reporting on a patient's mood, health, and wellbeing. Working under a regulated health professional you will be assisting in the maintenance of a safe environment for patients, staff and visitors, and maintaining effective communication with patients and health professionals.
