**Carefirst Seniors & Community Services Association** is a non-profit community services agency. Since 1976, we have provided community support, home and personal health care services in a holistic manner to seniors, individuals with disabilities, and those with medical needs in the Greater Toronto, York, and Mississauga regions. With a staff of 500 members (including full-time and part-time staff) and the support of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors and the physically disabled; 3,000 of whom are "home-bound" and frail. **TITLE: Dialysis Assistant** **TERM**: Full-time Permanent **RESPONSIBILITIES**: - Provide the individual with direct personal care, support and assistance for activities of daily living, dialysis education and recreation assistance in the clients home - Work together with other team members to maintain and enhance the physical, social, cognitive and emotional functioning by assisting the Integrated Dialysis Care (IDC) program participants to mobilize strength, recognize coping capacities, modify ineffective patterns of behavior, and accessing appropriate resources - Supervise and assist with personal care, personal hygiene, walking, transfer, movement and positioning, eating and feeding as needed **QUALIFICATIONS**: - Graduated from a recognized Personal Support Worker Program in Ontario - At least 1 year of working experience in community setting - Valid certification in First Aid & CPR, and Food Handling - Must have a valid driver’s license and be willing to commute to the patient’s place of residence - Ability to speak a second language that reflects the community served, eg. Cantonese, Mandarin, is an asset - Possesses a caring attitude, good interpersonal skills, sensitivity to recognize needs of clients and/or clients - Demonstrated good judgement, with the ability to work independently and cooperatively with the Integrated Dialysis Care team - Flexible working hours with availability to work shift hours, evenings, weekends, and statutory holidays - A valid vulnerable sector police record check and medical check completed within one year **COVID-19 Precautions**: - Carefirst has mandated a COVID-19 vaccination policy in place unless with exemption on a medical or other protected ground pursuant to the Ontario Human Rights Code._ - Carefirst is an equal opportunity employer.
