Reporting to the Unit Manager at the assigned organization under the guidance and direction of the Registered Nursing Staff. Is responsible for providing care, support, assistance, and supervision to patients in the assigned Health Care Centre; you will perform floor and observational care duties - helping to create an environment that promotes optimal physical, social, and emotional well-being for patients, working in collaboration with members of the patient care team.

**RESPONSIBILITIES**
- Participates with other members of the Care Team in promoting the health, well-being and independence of patients.
- Performs basic care procedures which she/he has been prepared for through the PSW Certificate Program
- Reports changes in patients’ physical, mental, and/or emotional condition to the Registered Nurse or designate.
- Immediately reports any problems or incidents involving patients and/or staff to the Registered Nurse in Charge.
- Receives verbal and/or written reports from outgoing shift members and documents incidents and conditions occurring on previous shift.
- Liaises and promotes positive relationships with the medical and facility staff, patients, families and visitors.

**QUALIFICATIONS**
- Completion of a Personal Support Worker certificate program or approved equivalent combination of education and experience.
- Up to one (1) year experience or practicum placement with resident care in a long-term care facility, hospital, or other geriatric environment.
- Police Vulnerable Sector Checks (PVSC).
- Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.
- Good interpersonal and communication skills to work within a diverse community and ability to deal empathetically with patients, family members, etc., to contribute to their well-being.
- Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
