Social Worker

Join our dynamic team of Occupational Therapists, Physiotherapists, Speech Language Pathologists and Social Workers involved in home and community therapy in Thunder Bay. We are hiring for an immediate opening.

Social Worker Job Responsibilities:

- Determines nature of client’s situation by interviewing client, assessing psychological, emotional and social information by making on-site / home visits.
- Explore options and setting goals with clients.
- Support clients through life changes related to health situation.
- Obtains assistance for client by referring him/her to community resources, arranging for appointments and establishing client support networks
- Fosters client action or adjustment through counselling and support
- Maintains clinical records by documenting client’s situation and client’s actions complying with PIR standards and the standards set out by the Registrant’s College.
- Monitors planned actions by periodic follow-up. Evaluates client progress.
- Maintains quality of service delivery by following policies and procedures, participating in quality reviews, and assisting with quality improvement.
- Complies with health, legal and company requirements.
- Maintains client confidence and protects personal health information as per company policies and PHIPA compliance.
- Contributes to team effort by accomplishing related results for client outcomes.

Qualifications:

- Bachelor Degree in Social Work
- Registration with the Ontario College of Social Workers and Social Service Workers
- Experience is an asset
