Summary:
Reporting to the Operations Leaders, Cardiac Sciences Program and working with the medical staff, Clinical Nurse Specialists, Access Leader and Patient Representative, the Cardiovascular Program Coordinator takes a leadership role in developing, implementing, evaluating and coordinating a cardiovascular waitlist management system. The Cardiovascular Program Coordinator as a key member of the Cardiac Sciences Program management team, plays an integral role in facilitating a patient-specific plan that establishes a streamlined process, timely and appropriate access and placement to necessary care, support and information to patients, families and referring Physicians waiting for cardiac surgery or intervention. The Cardiovascular Program Coordinator works closely with the BC Cardiac Registry and other Cardiovascular Program Coordinators at cardiac centres in the region and the province to ensure efficient and equitable access to cardiac services for all BC patients.

Qualifications / Skills and Education:
**Education**
Master’s Degree in Health Administration or Master’s Degree in a health sciences discipline and three (3) to five (5) years’ recent cardiac experience in a large, complex health care environment with expertise in program development and project management, or an equivalent combination of education, training and experience.

Duties and Responsibilities:
- Assumes a leadership role in collaboration with other Cardiovascular Program Coordinators and the BC Cardiac Registry in developing and implementing a cardiovascular waitlist management system. Develops processes and tools for ongoing evaluation and revision for the system, as necessary.
- Establishes systems and processes to assist referring physicians in triage of patients for cardiac catheterization, cardiac intervention, and cardiac surgery, within the institution, and between institutions to facilitate timely access to cardiac services by ensuring that referrals are dealt with in a timely manner and include complete demographic, clinical, and contact information, as well as relative urgency.
- Referring physicians are provided with accurate on-call and resource information to assist in referral decision-making.
- Referrals are forwarded to the appropriate cardiologist, angioplaster, or cardiac surgeon as per the institutional policy.
- Communication is facilitated between the health care providers involved in the patient’s care.
- In collaboration with the British Columbia Cardiac Registry, participates in development, implementation, and ongoing maintenance of a current, complete, and accurate database for all patients awaiting cardiac surgery at the Hospital, including tracking of patients waiting times and stability while awaiting cardiac surgery, cardiac intervention and diagnostic catheterization.
- Maintains current waiting lists, identifying patients exceeding recommended maximum waiting time.
- Ensures, in conjunction with the British Columbia Cardiac Registry, that statistics for waiting lists and cases completed are complete and recorded in the database in a timely manner..
- Establishes systems for notification of cardiac surgeons, cardiologists, and referring physicians of changes in patients’ conditions.
- Works with physicians to develop options to facilitate patients having procedures completed recommended waiting time guidelines.
- Provides patient/family centered support and educational information to patients waiting cardiac surgery, cardiac intervention or diagnostics by facilitating referrals to appropriate services (counseling, risk factor modification, and cardiac rehabilitation) internally or externally based on assessment of patient needs and services availability.
