We are currently looking for someone to join our Rehabilitation Team in the role of**Social Worker.**We are actively expanding our **Virtual Care Model** and are looking for professionals who want to be a part of this exciting growth. This role can be **located anywhere in Ontario**and provides oversight to the therapy assistants working directly with patients in the home.
**This is a Casual position.**
- HYBRID REQUIREMENTS:_ _Please note that this role requires you to be able and willing to travel in Ontario as needed for in-person appointments with our patients._

What The Role Involves:
- Professional assessment of patients in the community and in their homes
- Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Home and Community Care Support Services (HCCSS)
- Working with the patient on appropriate time-specific, measurable goals and outcomes in collaboration with the care team
- Working independently and as an integral member of an interdisciplinary care team
- Provide assessment, intervention and discharge planning, within the scope of practice
- Demonstrates a clear understanding of Community Resources, including funding
- Providing active guidance for workplace Health and Safety
- Performing other duties as required
What You Bring:
- Possess Master’s Degree (or equivalent) degree in Social Work from a recognized university
- Current, unrestricted license to practice with Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Strong assessment skills and the ability to participate in effective treatment programs
- Provide one-on-one therapeutic support with clients
- Promotes independence of the client in the home and in the community
- A reliable vehicle for some regular travel
- Excellent English verbal and written communication skills
- Clear Background and Vulnerable Sector Check
