Position: PACT Co-ordinator/Case Manager
Hours: 35 hours per week (hours will be flexible)
Location: New Westminster, B.C.
**Overview**:
The New Westminster Peer Assisted Care Team (PACT) pilot is a one-year program and will provide an alternative to the presence of police personnel at mental health crisis calls in New Westminster. The pilot teams will provide crisis support services that will integrate cultural safety protocols and holistic care options. Mobile teams comprised of a mental health professional and a peer (someone with lived or living experience) will be available during scheduled service hours under the supervision of the Co-ordinator/Case Manager. This program aims to reduce the harms experienced by people who have mental health and addictions challenges as well as Black, Indigenous, racialized, LGBTQ2+ and other marginalized individuals. The Co-ordinator/Case Manager will assess and support community members who are at risk of experiencing a crisis or are currently experiencing a crisis in the community, with the goal of providing culturally appropriate support, such as system navigation and timely referral to appropriate services, as well as providing short-term crisis intervention work such as counselling and case management.
**Reports to**:
The PACT Co-ordinator/Case Manager reports to the Acting Executive Director.
**Duties**:
- Supervises employees, provides information and direction on a daily basis
- Provides case management services and immediate and short-term, community-based follow-up support to people
- Assists in the development of client service plans to address immediate, intermediate and long-term client needs
- Oversees file management of client files
- Completes performance reviews on a predetermined basis
- Works cooperatively with staff and others to ensure safety policies and procedures are relevant and demonstrate high standards
- Monitors the safety of staff and their practices on a daily basis to ensure everyone’s safety and initiates crisis management protocols as required
- Coordinates shifts and schedules staff training
- Oversees program evaluation and determines outcomes and indicators for the program in collaboration with stakeholders
- Ongoing evaluation of day-to-day practices, making adjustments as needed to improve service delivery
- Gathers data for evaluation and reports to funders
- Works collaboratively with Canadian Mental Health Association (CMHABC) staff on all facets of the project
- Establishes relationships with key stakeholders including the City of New Westminster, the Police and Fire Departments, community organizations, the business community, resident associations, and others in order to ensure excellence in service delivery
- Establishes a Community Advisory Committee
- Completes the required paperwork for all aspects of the project
- Provides referrals to Purpose Society and other programs that can provide necessary services
**Qualifications**:
- A Master’s degree in Social Services or the equivalent from a recognized university
- A minimum of five years of community mental health and addictions experience with a strong knowledge of clinical and crisis assessments and interventions
- A minimum of three years in a supervisory position
- Knowledge of the mental health classifications, issues and literature
- Demonstrated skill in client-centred care, trauma and violence informed practice, skill development and counselling
- Understanding and knowledge of working with persons with mental health and/or addictions issues as well as an understanding of the intersectionality of systemic barriers and social determinants of health
- Knowledge of systemic and structural factors that impact Black, Indigenous, racialized and LGBTQ2S+ communities
- Experience developing and implementing new programs, including outcome evaluation practices
- Experience supervising employees
- Knowledge of common drugs, their effects, and the toxic drug crisis
- Advanced overdose recognition and response training
- Current First Aid and CPR certification
- NVCI Training
- Ability to maintain a non-judgmental and non-stigmatizing approach is required
- Willingness to participate in on the job on-going training and outside training
- Good verbal, written communication and problem-solving skills
- **A valid driver’s license is required, access to a car is desirable**
