West Parry Sound Health Centre (WPSHC) has always employed an innovative planning approach aimed at integrating services. Today our uniquely integrated health care organization includes: Lakeland Long Term Care, six Nursing Stations delivering care to rural communities, management of District ambulance services and dispatch, and a full range of in-patient and out-patient programs associated with a 90-bed acute care hospital.
WPSHC's innovative approach to integration has benefited the community through more seamless local delivery of services, improved access to primary care, a wider understanding of local needs and the ability to respond as these needs continue to evolve. **About the Opportunity**:
Reporting to the Clinical Manager, The Indigenous Navigator will provide support and advocacy for First Nation, Métis, and Inuit patients (in-patient, out-patient care), caregivers and families in the West Parry Sound Region. The position also provides the opportunity to contribute to program development, education and advocacy for systems change to address inequities facing First Nation, Métis and Inuit (FNMI) patients and families at the WPSHC and affiliated community programs.
The Indigenous Navigator position will provide culturally relevant support and advocacy for First Nations, Inuit and Métis patients and families accessing care within the WPSHC and affiliate programs, by facilitating and coordinating seamless care transitions across the healthcare continuum; addressing cultural/spiritual needs; and networking with FNIM and non-FNIM partners.

**Basic Requirements**:
- Minimum 3-year undergraduate degree in a health science, social science or Indigenous studies field or cultural recognized equivalent.
- Experience in Mental Health and Addictions Medicine; Trauma therapy training is an asset.
- Experience in Seniors’ mental health is an asset.
- Must have experience working with First Nation, Inuit and Metis (FNIM) peoples
- Experience working in partnership with First Nation communities and urban Indigenous organizations.
- Experience in addressing Indigenous specific health care issues.
- Previous involvement in program development, implementation and evaluation.
- Experience with collaboration and relationship-building with mainstream internal and external stakeholders.
- Experience in health care provision specific to Indigenous community members, and knowledge of the cancer care continuum as it relates to the Indigenous Path of Well-Being.
- Must have valid Ontario driver’s license, clean driver’s abstract, and access to a vehicle, as travel is required.
- Comprehensive knowledge of social work interventions, practice methods, and professional skills.
- Knowledge of other community resources, funding agencies, treatment centres, social service networks, and organizations.
- Skilled with communication on a one-to-one basis using appropriate grammar, and the ability to respond to difficult questions.
- Highly developed skills in the area of culturally relevant assessment and trauma informed therapeutic intervention with individuals, groups, and families are essential
- Extensive knowledge of FNIM health care needs from a cultural perspective
- Ability to navigate the Health Canada First Nation and Inuit Health Branch Non-Insured Health Benefits Program
- Demonstrated knowledge of and commitment to the health centre’s mission, vision, and values.
