The Glebe Centre is a leading Long-Term Care and Senior Outreach home that serves 254 residents and over 400 seniors in the vibrant Glebe community. - **Position Summary**:_

The Program Facilitator is responsible for the assessment of residents for the purpose of planning, developing, implementing, and evaluating a recreation / activities / therapeutic recreation program in accordance with MOHLTC standards and other applicable legislations. This position includes the supervision of students on placement and volunteers, as well as other duties and responsibilities assigned by the Director of Operations.
- **Applicants must have the following qualifications**:_
- A Bachelor’s Degree or College Diploma with formal training in Recreation Therapy from a recognized University/College.
- Affiliation with a Professional Organization (NCTRC, Therapeutic Recreation Ontario or Activity Professionals Ontario or equivalent).
- 1-2 years documented experience working with dementia and mental health challenges
- Strong leadership skills with an advanced knowledge of the aging process and dementia. Commitment to a multi-disciplinary team and a person-centered approach working with residents, clients, staff, families and volunteers.
- Excellent writing and problem solving skills long with strong organizational and interpersonal skills.

The following will be considered assets:

- Well-developed computer skills; experience in electronic charting including Point Click Care and RAI-MDS
- Experience in Palliative Care
