What You Will Be Doing:
You will be a welcomed member of the Sienna team, responsible for leading, developing and implementing improvements through site leaders that support operational excellence and/or align with the annual operating plan. As part of the regional team, reporting into Director Quality, the Quality Partner collaborates with regional partners to address priority initiatives, sustain best practices and identifying risk/ mitigate risk.
Key Responsibilities:
- Monitors, reports, and responds to clinical/non-clinical indicator performance outcomes in collaboration with regional partners
- Participates in assessing, reporting, and developing plans in response to existing and future risks at care community & regional level, assisting teams to resolve risks
- Support compliance action plans to enhance overall quality results for care communities and region, and to ensure sustainability
- Complete annual Quality Review in assigned care communities
- Ensure the individual care community’s annual Quality Improvement Plan (QIP) is complete and updated
- Coordinate and facilitate the scale and spread of Sienna initiatives and improvements across the sites.
- Leads implementing quality management policies within the region, including auditing systems and processes as per the quality and risk management program to: ascertain compliance; monitor system effectiveness; identify improvement opportunities and potential risk; and to validate improvement/change management initiatives
- Build capacity with site level teams to improve resident experience and quality of care and services
- Lead the accreditation planning & preparation process for the region, ensuring all required elements are prepared for survey including the follow up action plan to maintain award status

Who You Are:
- You have an undergraduate degree in Health Studies or equivalent and are proficient with Word, Excel, PowerPoint, Outlook, metrics reporting and have demonstrated commitment to customer service
- You have ability to exercise good judgment, discretion, and initiative in dealing with confidential information and/or in responding to inquiries.
- You have demonstrated ability in coaching, mentoring, training, and facilitating teams and individuals in lean, model for improvement, and other quality improvement methods; ability to do this at all levels of an organization (e.g. senior executive, mid-level managers, physicians, and front line staff)
- You are experienced with change management initiatives, ability to engage and involve stakeholders at all levels and acknowledge when quality targets are not met and develop action steps to make improvements
- You have demonstrated knowledge and understanding of the health-care system and issues, the unique needs and the challenges with across various settings: hospitals, long term care, and community organizations.
- You have proven ability to lead quality improvement projects, with a high competency in teaching and coaching others
- You have effective presentation and meeting facilitation skills including the ability to present and discuss complex information in a way that enables and establishes rapport, persuades and influences others and gains understanding.
- You have proven ability to work independently with mínimal supervision, take initiative
