**Annual Salary: $68,308 - $85,390**

Under the direct supervision of the Director of Care, the Behavioural Support Lead Facilitator is responsible for leading activities related to building knowledgeable care teams, and the implementation of evidence-based practices, research knowledge and expertise related to Behavioural Supports Ontario (BSO) into clinical practice within all care floors of Hogarth Riverview Manor.

**Qualifications**:
**Education/Experience**:

- A diploma in a relevant health science or related discipline
- Current registration as a Registered Practical Nurse with the College of Nurses of Ontario, without restriction preferred
- Minimum of three years experience in geriatrics, long-term care or community based seniors care
- Experience in program development, research and evaluation, community development, and education

**Skills/Abilities**:

- Ability to communicate effectively and concisely, both orally and in writing, with the ability to make presentations and deliver training to a variety of audiences
- Ability to establish and maintain effective relationships with employees, residents, families, and community partners, ministries and other external stakeholders
- Proven ability to function and contribute as an effective team member and to work independently
- Proven ability to provide coaching and mentoring
- Excellent project/time management, analytical/critical thinking, problem-solving, and decision-making skills
- Strong leadership skills including the ability to share skills and knowledge with others
- Must be proficient in the use of computers and current related word processing, spreadsheet, and presentation software
- Demonstrated regular attendance at work

**CONDITIONS OF EMPLOYMENT**
- Will be required to travel to various work locations and provide own transportation
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
