**Position Description**

Reporting to the Team Lead, the Adult Case Manager shall deliver culturally safe, trauma informed, clinical case management mental health and addictions services to their assigned First Nation community in the Treaty #3 territory. This position includes counselling with individuals, families and therapeutic groups, linking individuals to appropriate services and maintaining accurate client records. These services will be delivered within the policy guidelines of Kenora Chiefs Advisory and the mandate outlined by the Ministry of Health - Community Programs and Health Canada. The Adult Case Manager will work within the MHA program to help deliver and improve the services offered to the relevant communities.

Department: Adult Mental Health

**Salary**: $55,000-65,672 Per Annum

Term: Full-time Permanent

Location: 240 Veterans Dr, Kenora, ON P9N 3Y5

Reports to: Adult Mental Health and Addictions Program Manager

**Responsibilities**

Ø Provide intake screening on a rotating basis

Ø Develop an Initial Intake Assessment and create an individualized Care Plan

Ø Provide counselling, information, education and guidance to clients with regard to mental health and addictions

Ø Monitor clients' mental status and behavior for decline and take appropriate action

Ø Facilitate therapeutic groups. This could include: community sharing circles, therapeutic debriefing sessions, etc

Ø Participate in inter-disciplinary case conferences

Ø Keep accurate, timely client contact records in accordance with program requirements

Ø Assist and respond to crisis and post-crisis situations as required and complete individualized safety plans

Ø Facilitate linkages to needed services within the broader service community

Ø Develop networks and partnerships with community resources

Ø Advocate for clients and coordinate case management meetings with relevant partners

Ø Facilitate access to Anishinaabe knowledge, traditions and cultural beliefs in the healing process (if appropriate)

Ø Facilitate referrals to OTN for consults and assessments

Ø Maintain statistics required by the Ministry and the Program with regard to qualitative and quantitative aspects of the Program

**Qualifications**

Ø Ability to work independently and in a team setting

Ø Excellent skills in the following areas: computer, organization, networking, interpersonal, presentation and communication (written & oral)

Ø Experience working with and knowledge of Indian Residential School Survivors

Ø Knowledge of the Anishinaabe culture and language and experience in working in First Nation communities

Ø University degree or college diploma from a recognized post-secondary institution (Psychology or social work)

Ø Several years of counselling experience

**Additional Requirements**

Ø Valid driver’s license and vehicle

Ø Willing to travel (when required) to all Treaty #3 First Nation communities

Ø Current satisfactory criminal reference check with vulnerable sector search
