**Mental Wellness Case Manager (2 Year Term)**
**About The CMM**
**Position Overview**
Under the direction of the Director of Health and Social Services and reporting to the Wellness Programs Manager, the Mental Wellness Case Manager is responsible for the delivery of mental wellness case work and advocacy services to members of the mainland communities of Nova Scotia.
**Overview of Responsibilities**:
- Case Manage and advocate for Indigenous clients, while also keeping up to date client files and confidentiality agreements
- Work with mental wellness service providers to advocate for clients, as required, such as through letters of support for access to services and / or for legal reasons
- Inform clients of mental wellness service options and make referrals when necessary
- Provide advice and guidance to wellness team and community-based workers on complex behavioural needs and in developing case plans for these clients.
- Work with long term care facilities to ensure client needs are being met, as applicable
- Assist with navigating the provincial and federal mental health and social services programs for clients
- Support clients by developing/creating healthy coping skills and strategies
- Create and deliver mental wellness workshops and information for community members and community workers
- Submission of monthly report that includes a summary of the monthly performance of the program activities and the reconciled travel and other expenses against the annual budget
- Development and submission of an annual report summarizing all additional operational activities including the development and implementation of self-care plans, professional development activities and the updated status of the annual work plan
**Position Requirements**:
The following qualifications are desired:
- Undergraduate degree in health & social services-related field (examples: social work, nursing, child & youth studies, health & social services, etc.), or combination of relevant education, skills and experience
- Experience working in trauma informed care, mental wellness case work and / or in an advocacy role
- Minimum of 2 years experience
- Experience working with First Nation / Indigenous communities, First Nation service delivery organizations, and applicable government programming etc.
- Experience working in a fast-paced environment
- Understanding of Mi’kmaw and Indigenous culture, communities and organizations
- Knowledge of CMM member communities
- Working knowledge of First Nation Communities, First Nation service delivery organizations, and applicable government programming etc.
- Knowledge of available provincial health services and an understanding of First Nations Inuit Health of Indigenous Services and related programs and responsibilities
- Knowledge and working experience with the Truth and Reconciliation Calls to Action
- Understanding of the ongoing impact that Residential Schools have on survivors, descendants of survivors and communities
- Knowledge and understanding of Mi’kmaw health disparities and the impacts of the social determinants of health
- Ability to use culture as a foundation of healing and an understanding of how health systems impact Indigenous peoples and wellness

**Salary/Employment Term**:
Salary Range: $50,000 - $65,000 per year. This is a 2 Year Term Position.
