**Overview**
We are seeking a compassionate and dedicated Health Care Aide to join our team. In this role, you will provide essential support to patients in various settings, including assisted living facilities, nursing homes, and home health environments. Your primary focus will be on enhancing the quality of life for individuals by assisting with daily activities and ensuring their safety and comfort.
**Duties**
As a Health Care Aide, your responsibilities will include:
- Observing and reporting changes in patients' conditions to nursing staff
- Assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting
- Providing personal care services including catheter care and medication administration
- Supporting individuals with developmental disabilities and those requiring Alzheimer's or dementia care
- Monitoring vital signs and maintaining accurate medical documentation in compliance with HIPAA regulations
- Utilizing EMR or EHR systems for patient records management
- Engaging patients in meaningful activities to promote mental stimulation and emotional well-being
- Performing light housekeeping duties such as cleaning, laundry, and meal preparation
- Assisting with mobility using equipment like Hoyer lifts when necessary
- Providing hospice care to ensure comfort during end-of-life stages
**Experience**
- Previous experience in patient care, senior care, or long-term care settings
- Familiarity with medical terminology and procedures related to caregiving
- Certification in CPR and First Aid is preferred
- Experience working with individuals with disabilities or those requiring home & community care is a plus
- Strong communication skills and the ability to work collaboratively within a healthcare team
