The HRH/LHIN
**Integrated Manager of Transitions in Care **will directly lead identified staff whose function focuses on the scope of discharge planning and transitions in care from hospital. She/he will be the link connecting the two organizations to embed the philosophy of Home First in all relevant processes supporting seamless and timely transitions between hospital and community.

**Position Responsibilities**:

- Plans, manages, and evaluates the delivery of discharge planning processes and transitions in care from HRH.
- Accountable for effective team functioning in service and performance standards.
- Monitors and adjusts caseloads, workload and assignments within the hospital portfolio to maximize efficiency.
- Collaboratively with HRH Patient Relations Officer, manages complex and difficult patient issues and complaints with timely investigation and follow-up.
- May attend/support staff in Consent & Capacity Review Board hearings, service appeals and legal consultations.
- Acts as a key resource to HRH and LHIN staff, other managers, service providers and external stakeholders.
- Ensures that standards are maintained with respect to the assessment of patient needs, planning and coordination, to support patients and their caregivers
- Operationally, aligns with HRH and LHIN organizational directions, goals and performance standards at the team level
- Reviews the current status of ALC patients as required with the clinical teams and assists with problem-solving to support timely and supportive discharge.
- Monitors and manages resources as required according to established guidelines and HRH/ LHIN priorities.
- Assesses and identifies critical service delivery issues and risks to HRH and recommends and/or implements service improvements as required.
- Identifies and evaluates the hospital and community impact of proposed or implemented changes within assigned portfolio..
- Provides leadership in the development of communication materials to patients and families with respect to admission, hospital stay and discharge with appropriate stakeholders.
- Ensures patient services are provided in accordance with HRH and LHIN policies, relevant LTC Legislation, Public Hospital Act, other standards and guidelines.
- Creates mechanisms for improved communication, decision-making, and alignment between HRH and LHIN team members and the broader health system

**KNOWLEDGE**
- Demonstrated recent knowledge of HRH/Acute Care and Home & Community Care Support Services priorities, policies, practices and service standards
- Expert knowledge and understanding of legislation governing health care delivery
- Knowledge of direct care/case management models used in community health care
- Demonstrated knowledge of local health and community services
- Working knowledge of financial and statistical reporting
- Effective organizational skills allowing to prioritize and respond to multiple competing demands within defined time lines
- Proficient computer and business skills

**Qualifications**:

- Regulated Health Care Professional in good standing with respective health professional regulatory body in Ontario
- University degree in a health-related field, Master's level preparation an asset
- Master's level preparation in Health or Business Administration preferred
- A minimum of (5) five years of clinical experience in an acute care hospital
- A minimum of (3) three years management experience with direct people management responsibilities
- Exceptional analytical and information-seeking skills that contribute to effective decision-making and strategic planning and execution of action plans that deliver results and motivate individuals for greater performance excellence
- Ability to work with a diverse group of leaders and stakeholders with demonstrated success in moving key stakeholders to positive resolution and to facilitate and coach clinical areas on change initiatives
- Experience managing in a multi-union environment
- Track record of delivering exceptional clinical practice, efficiency and patient care
- Track record for creating a quality and safety culture and means for monitoring and communicating results that is meaningful to team members and inspires continuous improvement
- Self-directed, courageous, and highly motivated with excellent interpersonal and communication skills
- Experience managing competing demands in a fast-paced, multi-site, multi-disciplinary health care environment
