**Full Time SOCIAL WORKER**

The Salvation Army Toronto Grace Health Centre (TGHC) provides medically complex, and specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care and Integrated Transitional Services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals in returning to their community.

Reporting to both the Allied Health Manager at TGHC and the Manager of the Specialized Care Centre
- Scarborough location.

The Social Worker, as a member of the interprofessional team, provides safe, quality, patient centered care to individuals with chronic and terminal illnesses including providing support to their families and significant others. The Social Worker maintains a link between the patient, family and community to support the interprofessional care team’s individual patient care plan.

**MAIN RESPONSIBILTIES**
- Provide social work support services to patients/families depending on current need. This could include complex continuing care, rehabilitation, and palliative care or transition to a community. Patient population could include vented patients, acquired brain injuries, bariatric individuals.
- Provide a range of social work services including psychosocial assessment, discharge planning, counseling to patients and their families and crisis intervention.
- Act as liaison and assist patients during their hospital stay and as they transition to their home environment
- Provide support in hospital and in the community as patients transition to their home environment
**QUALIFICATIONS AND EDUCATION**:

- Master’s degree from an accredited School of Social Work as recognized by the Ontario College of Social Worker is required
- Member in good standing with the OCSWSSW.
- Flexible schedule is required.
- Experience in a hospital or health care setting.
- Experience with discharge planning
- Knowledge of the acute, sub-acute and community sector.
- Knowledge of community resources is an asset
- Previous experience working with seniors and those at risk
- Experience in mental health is asset
- Knowledgeable of cultural system perspective that includes family, community, religious and spiritual resources
- Demonstrate ability to welcome change and continuous improvement
- Strong communicator with proven problem solving and interdisciplinary skills
- Proven ability to function effectively within an interdisciplinary team
- Excellent interpersonal skills as demonstrated by your ability to develop and maintain positive relationships with patients/families, and members of the health care team
- Demonstrated facilitation and problem solving skills
- Exceptional time management skills
- Excellent communication skills (oral and written) facilitating effective communication in case reporting and assessments, with patients, families, other healthcare team members and volunteers
- Self-motivated, team player, and ability to thrive in a fast-paced environment

**Salary**: $37.00-$45.00 per hour
