**Job Summary**
The **Personal Support Worker/Caregiver**is a member of the home care team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the client’s needs.

We are looking for PSW who is available
- Italian Speaking PSW or Companion
- Available **2-3 times a week from 2 pm until 6 pm**, minimum twice a week. Either Monday and Friday or Tuesday and Thursday
- Near Jane and Finch

**Responsibilities**
- AM and PM care
- Meal Preparation
- Light Housekeeping
- Accompany clients on outings
- Shopping and errands
- Medication Reminders
- Ambulation and transporting of clients
- Provide companionship to clients
- Light Housekeeping
- Ability to follow a care plan
- Documentation/Charting

**Qualifications**
- Personal Support Worker Certificate or equivalent (eg. foreign-trained nurse, nursing student after one year of completing the school).
- Current CPR/First Aid training
- Current Police Record Check or Vulnerable Sector Screen or be willing to obtain one.
- Negative TB skin test or chest x-ray
- Access to the internet/mobile phone.
- Great support and teamwork
- Dementia/Alzheimer experience
