**Summary**

We are seeking an MDS Coordinator to join our dynamic team at Help Now Healthcare. This role is essential in coordinating assessments and ensuring compliance with healthcare regulations while providing exceptional support to our patients and their families. The position offers the opportunity to work in a flexible environment that values continuing education and professional growth.

**Responsibilities**
- Coordinate the completion of MDS assessments for residents in nursing homes and long-term care facilities.
- Ensure compliance with provincial and federal regulations regarding patient assessments.
- Collaborate with interdisciplinary teams to develop individualized care plans based on assessment findings.
- Provide training and support to staff on MDS processes and documentation requirements.
- Monitor quality indicators related to MDS assessments and report findings as necessary.
- Assist in the development of policies and procedures related to MDS coordination.

**Requirements**:

- Experience in nursing homes or acute care settings is required.
- Strong knowledge of case management principles and practices.
- Familiarity with Cerner or similar electronic health record systems is an asset.
- Excellent communication skills for effective collaboration with healthcare teams.
- Ability to manage multiple priorities in a fast-paced environment.
- Experience in dementia care or pediatrics is a plus.

Pay: $34.00-$40.00 per hour

Expected hours: 40 per week
