Model of Care Implementation Lead

The Director, Professional Practice is currently seeking a Model of Care (MoC) Implementation Lead. The purpose of the position is to provide operational leadership to the MoC Transformation strategic project at Waypoint. The MoC Implementation Lead will be the primary contact for day-to-day activities associated with the implementation of the MoC work. Working in close coordination with Director, core project team, and unit-based teams, including service recipients, the incumbent will coordinate the development and implementation of work plans for the duration of the MoC transformation project, ensuring that all stakeholders’ needs are considered in outcomes.
**Key Accountabilities**:
- Provide day-to-day oversight to the implementation of Waypoint Centre for Mental Health Care’s MoC project as a key component of Waypoint’s strategic plan - including but not limited to coordinating, planning, managing and executing implementation work plans.
- Serve as the key project resource across all implementation teams and working groups to ensure alignment to the established vision and principles of MoC while ensuring alignment of work streams
- Identify resource requirements (people, financial) to support MoC implementation and work with implementation leads to secure necessary supports - escalating to the Project Director as required.
- Facilitate local problem-solving at all phases
- Support and enable the active participation of Patient/Client and Family Council Members in the co-design of the MoC
- Provide direction, coaching and mentoring for unit-based implementation leads
- Actively support and facilitate capacity building for implementation across the organization
- Participate in key committees supporting the MoC project governance structure
- Prepare project related communication, including reports and presentations
**Requirements/Qualifications**:
- You must meet the requirements of our **COVID-19 Immunization Policy** by providing proof of full vaccination, or a request and receipt of an accommodation under the Ontario Human Rights Code, including a medical exemption
- Post-secondary degree, certificate or diploma, in healthcare or business administration field or comparable work experience required.
- Project management / change management certification, and formal quality improvement training required.
- A minimum of 5 years’ experience leading corporate transformation initiatives in a health care environment required.
- Health professional designation preferred
- Demonstrated knowledge of health care regulatory frameworks, models of care delivery, and interprofessional scopes of practice
- Experience working within a matrix management model, providing direct oversight in a supervisory capacity
- Strategic and project planning abilities, “out of the box” thinking and ability to take prudent risks.
- Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
- Demonstrated leadership, conflict resolution, interpersonal and organizational skills
- Proven written and verbal communication skills
- Proven ability to adapt readily to change
- Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation), and reflects an optimistic and positive attitude
- Preference for proficiency in French/English language skills
