**The Role**

The Patient Coordinator oversees the patient experience, and serves patients by being their first and last point of contact. This is an integral role that works with all members of the healthcare team in a coordinated and collaborative effort to engage, encourage and support patient health. This role may start in a remote capacity initially, and will transition to an in-person role in order to facilitate the opening of a new clinic in Brampton, ON. The key responsibilities of this role include, but are not limited to the following, and will vary based on the needs of the Aroga from day-to-day.

**Responsibilities**:

- Welcome patients as they arrive at the clinic, and direct patients during their visit. Explain consent form to patients; answer any questions and record consent.
- Assist with bookings for patients and explain the importance of the visit.
- Educate patients, and/or caregiver attending the appointment, on what to expect from the appointment and what/how to prepare for the initial assessment.
- Coordinate the flow of patients in the examination and treatment areas to ensure efficient use of the facility and the practitioner's schedule.
- Prepare examination rooms with necessary medical supplies (if applicable).
- Provide a variety of administrative duties within the clinic such as data entry, faxing, scanning, shredding, dictation letters, photocopying, preparing patient information booklets and other related tasks, as required.
- Receive, scan and distribute incoming mail, labs, consults, prescription requests, special authorization requests and diagnostic imaging reports to the team.
- Support the improvement of pre
- and post-encounter processes (eg., identify missing vitals, lab results or screening needed for patients with appointments).
- Ensure the protection of patients' rights by maintaining confidentiality of personal and financial information.
- Process incoming referrals to physician providers.
- Call referring physician’s office for updated patient demographics and health care number.
- Contact referring physician office, clinics, hospitals, labs, diagnostic centers or specialists for additional clinical information as needed.
- Arrange new referrals and follow-up appointments.
- Create patient appointment notification letters to referring physicians for all appointments as required. Alternatively, create fax letters to referring physicians for missed appointments, inability to attend, cancellations, refusals or specific letters pertinent to patients visits.
- Involved with physician scheduling.
- Maintain inventory of medications and follow all procedures regarding outdated medications and other supplies.
- Assist in the training of new clinic Patient Coordinators staff in shared duties.

Qualifications:

- Previous administrative and/or customer service experience is considered an asset.
- The role requires excellent interpersonal communication skills, both in person and over the phone.
- Ability to handle a high volume of work and work well under pressure.
- Ability to address patient concerns and challenges with compassion and resilience.
- Ability to handle sensitive patient information with confidentiality and discretion.
- Ability to speak Punjabi and/or Hindi is an asset.

**Job Types**: Full-time, Part-time

Schedule:

- 8 hour shift
- Monday to Friday
- No weekends
