**JOB TITLE: Health Guide** **PROGRAM AREA: Integrated Programs - Hive Health Services** **EMPLOYMENT STATUS: Full-Time, Ongoing** **HOURS/WEEK: 35** **Our Vision** A community without barriers to health and wellbeing. **Our Mission** We reduce health inequities by providing interprofessional primary health services and community programs, focused on the populations we prioritize, in collaboration with community partners. **Our Values** Innovation, Accountability, Person-Centred, Health Promotion, Excellence, Sustainability, Accessibility **Guelph Community Health Centre prioritizes people who face barriers to health and accessing services, and people who identify as**: - Homeless or at risk of homelessness - Living on low income - Newcomers to Canada with language barriers - Having moderate to severe mental health and/or addiction issues - Having moderate to severe disabilities - Indigenous First Nations, Inuit and Métis - Black, mixed race and racialized populations - Experiencing intersectional oppression - 2SLGBTQIA+ - Vulnerable children and their families experiencing conflict, isolation or attachment struggles Working as part of an interdisciplinary team to provide clinical health services for people with HIV/AIDS and people seeking gender-affirming care, the **Health Guide** supports the coordination of care for Hive Health Services Clients. This coordination includes leading the development of a client-centred care plan of existing supports, as well as providing assistance to clients experiencing difficulty accessing resources and navigating systems. The Health Guide facilitates internal and external referrals, engages in problem-solving and advocacy, assists clients in developing strength based coordinated care plans and collaborates well with service partners. **Main Responsibilities** - Actively engages clients, interdisciplinary team members and external partners in the development of a client-centred coordinated care plan to support Hive Health Services clients. - Assists clients to reach their identified health goals, acting as a resource to navigate social service and healthcare systems and ensuring that they have the supports they need to optimize their health and wellbeing - Works from a determinant of health and client-centred perspective in order to assist in building clients' capacity to work toward goals. - Provides active engagement opportunities and support for complex clients who require extra support. - Advocates for clients to receive the appropriate social service and health care. - Facilitates internal and external referrals. - Establishes and maintains knowledge of community resources and builds strong relationships with external agencies and partners. - Maintains electronic client records according expectations and professional standards. - Meets regularly with supervisor to keep them fully informed on program developments, issues, activities and recommendations of any program changes. **Key Qualifications and Skills** - University degree or college diploma in social work or other relevant discipline, or an appropriate combination of education and experience. - Three to five years’ experience in a community based organization - Experience working effectively with culturally, economically and socially diverse clients. - Familiarity with social service and health programs, services and systems in Guelph and Ontario. - Experience working with people with HIV/AIDS, people who identify as Transgender and people seeking gender affirming care considered an asset. - Demonstrated expertise in client-centred, trauma-informed care and understanding of the value of interdisciplinary care in a health care setting. - Expertise in working with clients with mental health and addiction issues. - Excellent English written and oral communication skills. Second language is an asset. - Proficiency in the use of computers and various software programs. - Possession of a valid Class "G" Ontario Driver's License and access to a vehicle. - High level of confidentiality - Flexible towards change - Understand and are committed to the Mission, Vision and Values of the Guelph CHC.
