**Overview**
The program works closely with Fred Victor’s Addiction Supportive Housing (ASH) with 4 case managers who work closely to support program clients. The two programs manage 32 units of rent-supplemented housing, and several individuals receive only case management support. The case managers provide intensive case management to support 9 to 14 clients.

Our work is grounded in the lived experience of the clients and the communities we serve, understanding of social determinants of health and the principles of anti-racism, anti-oppression, equity, excellence. We are an agile team that promotes learning and innovation. We provide high quality case management services and housing stabilization services to individuals experiencing homelessness in the community and dealing with severe & persistent mental health difficulties.

**Duties**
Provide both on-site and community-based assessment and long-term case management services to individuals living in units in various locations. Services are short-term and intended to connect individuals to needed services to support their ongoing wellbeing.

Provide intensive case management to support clients to stabilize their living situation. Identify client’s goals around substance use, mental health, and treatment, which are all part of a comprehensive treatment planning process which utilizes a harm reduction approach.

Supports clients in the acquisition of skills in daily living, developing coping strategies and promoting new behaviours that improve wellbeing and safety.

Ongoing screening, assessment, monitoring, referral, advocacy, counselling, education, and life skills training are all part of this program.

Case Managers work as part of a team to meet clients’ housing and other social and health needs. The program works closely with a variety of health care professionals & coordinates services in various settings including, but not limited to, hospitals, housing, incarceration systems, various treatment settings, etc.

**Skills**
**What You Bring to the Team**:
Post-secondary education with a degree or diploma in Social Work, Social Services, or Health.

A minimum of two years of experience within the homelessness services sector, preferably in concurrent disorder services with an emphasis on providing support through a recovery and strength-based approach.
- Working knowledge of the Housing First, Harm Reduction, and Trauma-Informed frameworks.
- Engaging effectively with individuals experiencing serious mental health issues and/or addictions, cognitive difficulties, brain injury or primary health care issues.

- Works from a harm reduction approach with clients dealing with addiction issues.
- Exceptional ability to work cooperatively with team members, hospital personnel, other service providers and other stakeholders to better meet client’s needs.
- An understanding of the Mental Health Act and the processes involved in intervening in an unsafe situation related to an individual’s mental illness.
- Documents client-related activities accurately and in a timely fashion on the Client Information System’s database
- Excellent interpersonal skills, sound knowledge of counseling skills and the ability to form relationships with marginalized individuals.
- Strong understanding of and commitment to social justice issues, equity issues, anti-racism, and anti-discrimination practices.
- Excellent documentation skills and the demonstrated ability to document regularly and within expected time frames for the program.
- Highly developed understanding of and proven ability to communicate appropriate professionally boundaries.
- Ability to work from a trauma informed/client-centred approach.
- Demonstrated ability to follow policies, procedures, and adhere to legislation.
- Understanding of clinical practices and care models.
- Ability to exercise good judgment.
- Applied computer skills (Microsoft Word, Excel, and Internet).
- The ability to communicate in another language is an asset.

Pay: From $28.68 per hour

Expected hours: 37.5 per week
