**Job Description**:
**Position Summary**:
Facilitating continuity of care, the Social Worker, provides assessment, counselling and treatment services to patients in an acute, critical care and outpatient clinic settings. The Social Worker assists patients and families address the impact of illness and treatment; tremendous stress often stems from hospitalizations that are sudden and at times related to catastrophic illness or injury.

Social workers provide assessments and appropriate interventions to aid the patient in achieving optimum recovery and quality of life. The social workers at Cambridge Memorial Hospital have expertise in the areas of general medicine, surgery, emergency, geriatrics, critical care, oncology, palliative, obstetrics and paediatrics. The Social Worker will also engage in resource linkage and service coordination, consultation and collaboration with internal and external service providers in order to optimize the health and functioning of clients and their families.

**Skills and Qualifications**:

- Masters preparation and registration with the Ontario College of Social Workers and Social Services Workers Social Work (MSW, RSW) required.
- An in-depth working knowledge of the psychological impact of chronic illness on the person and the family.
- Working knowledge of the health system and relevant legislation (i.e. Mental Health Act of Ontario, Child & Family Services Act, Health Care Consent Act) is required.
- Ability to provide crisis assessment and intervention, including suicide risk assessment, prevention and intervention is required.
- Strong skills in community resource linkage, and collaborative and innovative care planning within a multidisciplinary team is required.
- Proficiency working on a team of multidisciplinary healthcare professionals is required. This includes strong verbal and written communication, mediation and teamwork skills.
- Demonstrated ability to attend work on a regular basis is required.- The successful external applicant is required to provide the applicable criminal record check, as requested by Human Resources, in accordance with federal and provincial legislation. Valid criminal record checks are in original format and dated with two months from date of conditional offer from CMH._
