Housing Worker-intensive Case Manager-housing Loss Prevention

Summary of Duties
The Halton Housing Help Intensive Case Manager - Housing Loss Prevention provides support services to clients who are having trouble accessing or maintaining housing. This position is part of the Halton Housing Help team and reports to the Manager of Halton and Housing Help.
Key Responsibilities
- Utilize a customized risk assessment tool grounded in the social determinants of health to identify high risk households and tailor interventions
- Provide intensive case management to individuals and families at risk of housing loss, focusing on upstream prevention
- Develop and implement housing stabilization plans in collaboration with clients, focused on avoiding eviction, maintaining tenancy, or securing alternative housing.
- This can include: Repayment planning, Housing search, Landlord mediation,Advocacy to preserve tenancies
- Offer support through mediation, system navigation, and referrals to community resources (e.g. income supports, legal services, health care, employment programs, recreational programs) to foster community connection and belonging
- Network and liaise with community and government social services, enforcement and housing agencies and networks to support clients
- Act as an advocate for clients on housing issues with social and government agencies as appropriate
- Work closely with community partners to coordinate early intervention supports
- Provide continued assistance beyond the crisis period to help maintain stable housing and long-term outcomes
- Work with Regional staff to understand client by-name data history for more targeted interventions
- Monitor client outcomes and contribute to data collection and reporting requirements set out by the program funder
- Identify and analyze clients’ needs and respond with a plan of action in collaboration with the client
- Assess clients’ situation to determine if they may qualify for priority status with Halton Access to Community Housing (HATCH) and complete necessary documentation
- Collaborate and link client with appropriate community agencies and/or groups
- Maintain client records, reports and statistics
- Perform other duties as assigned
**Qualifications**:
Education & Experience
- Post Secondary degree (B.A., B.Sc., B.S.W.) or approved equivalent
- 2-5 years community based experience
Skills & Knowledge
- Administrative and organization skills with the ability to prioritize tasks
- Ability to maintain accurate and clear client records
- Communication skills (verbal and written) in order to clearly and concisely exchange information with a wide variety of contacts
- Interpersonal skills to effectively liaise and consult with clients, families, other team members and community contacts
- Interviewing, assessment and objective observational skills
- Ability to work both independently and within a team environment
- Ability to manage a case load in terms of priority and controls
Knowledge of:
- Relevant legislation and policies including the Residential Tenancies Act
- Community and housing resources
- Housing, poverty issues and other social issues
- Issues related to mental health or seniors is an asset
- Computer-based work processing and database entry
**12 Month Contract Full Time Position**
**Starting Hourly Rate: $23.72/hour**
**Earliest Anticipated Start Date: Monday September 8th 2025**
**Hours of Work: Monday to Friday, 9:00am-4:30pm**
**Location: Primarily working throughout Halton Region. Extensive car travel required.**
