Shelter Now is a non-profit charitable organization which is recognized as a leader in the development of solutions to homelessness. Shelter Now helps transition homeless people to self-sufficiency while contributing to a healthy, vibrant community by ensuring that the most vulnerable have access to safe, affordable housing and supports. We currently operate at Hartog House which is a 20-unit transitional housing complex and a new development of 32-Supportive Housing Units in Midland, Ontario.
The Community Support Worker (CSW) is responsible to provide Case Management support with a housing focus to those who reside in the supportive housing program. Offering support with implementation, coordination and evaluation of Individual Support Plans, as well as assisting clients with day-to-day support, while contributing to facilitate on-site programming to develop life skills for independent living. The program is a goals-based program, addressing barriers to housing and overall well-being.
**Duties and Responsibilities**:

- Working directly with and reporting to the Team Lead.
- Completing the intake and admission process including orientation to the program, while working collaboratively with the Shelter Now Team to determine eligibility and feasibility for service.
- Meeting regularly with clients including home visits to provide direction and support, attend appointments with clients as necessary. Collaboratively identifying program needs and assisting with the implementation of ISP and responding to daily client service needs.
- Working with the Shelter Now team, developing and delivering group programming and creating monthly calendars and outsourcing guest speakers and facilitators.
- Engaging community resources to support client services. Assisting residents in identifying and obtaining relevant resources and engaging family and community supports as may be needed to achieve the goals. Potential supports might include mental health services, vocational and employment counselling, life skills training, financial planning, health education, faith groups, volunteer, literacy training, addictions services, assertiveness training, recreational services, etc.
- Working as part of a team with the Transitional Rehabilitation Housing Program (TRHP) in partnership with Waypoint Centre for Mental Health, and Canadian Mental Health Association. Maintaining all working relationships with partnering agencies associated with Shelter Now.
- Providing coaching and training needed to implement the ISP. Services may be offered in-house or outsourced as determined by the support team.
- Maintaining current documentation of the service activities, case notes, monthly statistics, and database entries.
- Coordinating and planning discharge process in collaboration with the Shelter Now Team.
- Organizing and coordinating donations of furniture and household items for distribution to clients as needed.
- Evaluating and making recommendations to ensure excellence in program service delivery.
**Skills and Qualifications**:
- Degree/diploma in Social Services: BSW or SSW
- Registration with the Ontario College of Social Workers and Social Service Workers as an RSSW
- Minimum of 3 years of experience working in the field of social services
- Experience preferred in working with vulnerable and at-risk individuals with disabilities, mental illness, homelessness and poverty
- Working knowledge of community resources
- Physical demands associated with occasional lifting of office supplies and other materials
- Position works collaboratively with other care team providers
- Computer literacy: Microsoft Suite
- Valid Driver’s License
- First Aid/CPR
- Criminal Reference Check with Vulnerable Sector Screening

**Payscale and Benefits**:
Salary starting at $53,000 annually plus benefits and matching RRSP contributions up to $1500 annually.
