Community Support Practitioner

The Bluefox Association is a private non-profit organization that provides residential and vocational support services to adults with developmental disabilities.
Job Summary:
The Community Support Practitioner is a frontline role that supports individuals with disabilities whom have complex needs in their community and homes.
Key Responsibilities:
Assisting and supporting individuals to participate in a meaningful way in their personal life planning - activity planning in community (goal oriented activities and recreational activities)
Assisting, supporting and encouraging individuals to achieve their personal goals through the development of basic living and related skills with a view to enhance the individual’s confidence, competence, and participation within their social network (teaching daily living skills, basic daily requirements, etc.).
Maintain clear and objective written records and required paperwork
Assisting and supporting individuals in their home
Role modeling / coaching
Communicating in a professional, timely, accurate and respectful manner with individuals, families, guardians, employees, and community representatives.
Working effectively in a collaborative, team-orientated environment.
Daily documentation (logs, data, Medication Administration, etc).
Qualifications:
Clear Criminal Record Check / Vulnerable Sector Check
Standard First Aid & CPR Level C
Positive attitude and belief that persons with disabilities have the right and responsibility to define and pursue their life choices and pursue active participation and involvement in the community.
Strong verbal and written communication skills
Ability to work as a collaborative team member
Effective time and priority management skills, flexibility and adaptability
Well-developed observation and interpretive skills, ability to identify observable changes in behaviour and attitude
Ability to balance competing demands and priorities
**Hours of work: There are different schedules that we are looking to fill; please indicate the dates and times you are available to work. Weekend availability to work shifts is required.**
Starting Wage: $18 per hour
**Job Types**: Full-time, Part-time
Pay: From $18.00 per hour
