**Whole Health - Care Coordinator**

Provide support and oversight of medical appointments and therapies for an individual who has a care plan in place to address a long-time seizure disorder - lives in their own home, in West Pickering and has their own dedicated group of people who assist with daily support needs.

Looking for someone with Knowledge in various health conditions - including epilepsy. Whose role will be to efficient at maintaining good organization of health and medical related records and documentation in a family friendly manner. Although this is a position of coordination, it requires the person to also have the ability and comfort of offering good personal support to the person of focus.

**Duties include but are not limited to**:

- Liaison between health resources, family and supporters.
- Attend medical appointments to support family, both in the getting to and being at appointments and assisting with keeping notes of appointments.
- Assist with and provide support to follow up on all medical appointments.
- Attend a monthly family meeting to assist with providing general updates on status of appointments, medications and therapies.

**Qualifications and Requirements**:
Experienced in-home care and epilepsy. Nursing background would be an asset.

First Aid and CPR certified

Current Vulnerable Sector Check

2M in Commercial & General liability Insurance.

25K in Abuse liability

**Job Type**: Part-time

Pay: $30.00 per hour

Expected hours: 7 per week

Schedule:

- Day shift

Ability to commute/relocate:

- Pickering, ON (L1Y): reliably commute or plan to relocate before starting work (required)

**Experience**:

- Epilepsy: 1 year (required)
- Nursing: 1 year (preferred)
- Home care: 1 year (required)

Licence/Certification:

- First Aid Certification (required)
- CPR Certification (required)
- Vulnerable Sector Police Check (required)
- Class G Licence (required)
