We are currently seeking a permanent full time Health Care Worker - Adult program to join the Mental Health and Substance Use Intensive Case Management (ICM) team at the Penticton Health Centre.

**What will you work on?**
ICM is a team-based model of care, serving individuals with moderate to severe substance use, who may, or may not, have a mental illness and/or be homeless. The team provides health and social services within the community setting, and provides an outreach component to work with this vulnerable population to provide them with services to address their substance use, mental illness, general health and other needs.
The Health Care Worker - Adult Program assists clients with mental, developmental and/or physical disabilities to live successfully in the community by determining appropriate care to meet individual needs and situational requirements. Provides a variety of day to day physical, emotional, and social supports, advocacy, life skills assistance, information, resources and demonstrations to enable independent living within a range of housing options. This position is administratively responsible to the Program Manager, Adult Mental Health Services.
**Scheduling Information**:
Shift types are days and shift times are from 08:00 to 16:00 hours.

Education, Training and Experience:
- Grade 12
- Human Service or Rehabilitation Assistant Diploma plus a Community Mental Health Certificate
- Class V BC Driver’s License
- Certificates in CPR, First Aid, and Food Safe
- Recent related experience of two years’ working with people with serious and persistent mental illness in the community. Experience working in a collaborative multi-disciplinary and multi-agency context supporting persons with mental illness.
