Home Instead is Nova Scotia's trusted source of non-medical care services for older adults who wish to remain living at home. We are looking for a motivated, results-oriented professional for the role of Client Care Coordinator who can conduct in-home care consultations, perform quality assurance visits and otherwise manage our client accounts to provide the highest quality service to our senior clients and their families.

**Primary Responsibilities**:
Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.Conduct Service Inquiries and Care Consultations as needed following the consultative sales process.Work with other team members to coordinate various aspects of a client's care.Conduct client/Care Professional introductions with every new client/Care Professional.Create and maintain client and responsible party records documenting all quality assurance meetings.Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.Recognize and pursue opportunities to introduce Home Instead services to RPNs while visiting apartment buildings, condos, assisted living communities and the hospital (superintendents, managers, care managers, nurses, social workers etc.).Generate weekly, monthly and quarterly performance reportsParticipate in community events and networking events as necessaryConduct care consultations/intros and other on call tasks every 4th to 5th weekend

**Education/Experience Requirements**:
CCA, or a professional in a health-related field2+ years of relevant experienceMust possess a valid driver's license and a reliable vehicleAbove average written and verbal communication skillsStrong administrative and computer skillsCompetitive spirit, capable of working independentlyStrong relationship building skillsSelf
- motivated with excellent prioritizing and time management skillsProven ability to negotiate, influence and problem solveThe flexibility to work in a 24/7 environment where work outside of office hours can be required
