The Mental Health Counselor/ Social Worker is responsible for working with physicians and other members of the Family Health Team to assist individuals with mental illness and psychological issues. The Mental Health Counselor/Social Worker will communicate with other mental health agencies in the community to ensure that patients are referred to appropriate programs and services.

**Operational**

a) Assess patients’ mental health needs; determine the most appropriate treatment plan for the patient.

b) Develop, maintain and facilitate workshops and ongoing groups for wellness and positive mental health.

c) Provide counselling to people who are experiencing difficulties in their relationship.

d) Counsel patients on mental health concerns; assist them in understanding their mental health and improving their overall health.

e) Conduct assessment and treatment of cognitive, emotional, behavioural disturbances by psychotherapeutic means; delivered through a therapeutic relationship based primarily on verbal or non-verbal communication.

f) Facilitate group counselling.

g) Support the use of assessment tools that other members of the Family Health Team can use to assess the mental health condition of patients.

h) Remain current with the latest mental health research and trends.

i) Create and/or update patient charts to document patient progress.

j) Collaborate and consult with community groups and other health care agencies to develop new public education programs and identify and fulfil community health care needs.

k) Prepare documents, records, and reports as required.

**Health and Safety**

a) Personally comply with and assume appropriate supervisory responsibility for compliance with all health and safety practices in the organization; in accordance with policies, standard operating procedures/guidelines, and the Occupational Health & Safety Act and applicable regulations.

b) Promote and support a culture of patient safety and advocacy.

**Other**

a) Comply with all policies and procedures of the Listowel-Wingham & Area Family Health Team.

b) Perform all other duties and responsibilities as assigned.

c) Recognize, understand and uphold the FHT’s Vision, Mission and Values.

**Education/Training/Certifications**:

- Master’s Degree in Social Work, Psychology or equivalent.
- Registered with the Ontario College of Social Workers and Social Service Workers or the College of Registered Psychotherapists of Ontario or the Ontario division of the American Association for Marriage and Family Therapy or comparable organization.

**Experience**:

- Minimum one year of related experience.

**Skills and Abilities**:

- Thorough working knowledge of social work/psychology/psychotherapy principles and practices.
- Excellent critical thinking and problem-solving skills.
- Strong interpersonal and customer service skills.
- Exceptional organizational and verbal and written communication skills.
- Ability to demonstrate tact and discretion in handling confidential and/or sensitive matters; maintain confidentiality.
- Ability to effectively foster good rapport and cooperative working relationships; and to champion the corporate mission and values.

Demands and Working Conditions

Work is typically performed in a standard office setting. Extensive computer work including, keyboarding, concentration, data entry, and sitting for extended periods of time, is required.

The Mental Health Worker/ Social Worker is required to make decisions and solve problems using judgement and interpretation. Frequently exposed to human suffering and loss of life. Interaction with clients that may exhibit violent or harassing behaviour, have special needs, or mental illness.

**Job Type**: Part-time

Pay: $35.60-$40.60 per hour

- French not required

Schedule:

- Day shift
- Monday to Friday
