The Quality Improvement Specialist provides support and guidance regarding regulatory and compliance issues and best practices for community and home health care. The Quality Improvement Specialist is responsible for the coordination, management, and development of the quality assurance activities required to support, evaluate, audit, document and control of all quality assurance functions throughout the production cycle. This role provides leadership to support internal and external quality and performance monitoring and reporting. Also, the Quality Improvement Specialist provides leadership in the evaluation and ensuring client satisfaction. This position provides an objective, independent view of the quality of services and the execution of procedures.

**What a day looks like in this role**
- Manages and coordinates organization-wide efforts to ensure that performance management (PM) and quality improvement (QI) programs are developed and managed using a data driven focus that sets priorities for improvements aligned to ongoing strategic imperatives.
- Implements a performance management tool and improvement process that leads to a positive and measurable outcome.
- Alongside organizational leaders, supports the development and implementation of organization initiatives.
- Contributes to driving a continuous quality improvement and client safety culture.
- Researches and designs appropriate organization-wide performance and quality training.
- Identifies improvements to on-boarding/continuous training and coaching to staff when needed.
- Lead CARF Accreditation and ensures continuous conformance with CARF Standards.
- Participates in Monthly, Quarterly and Annual reporting to RNS stakeholders, including but not limited to, Home and Community Care Support Services, Children’s Treatment Network, Region of Waterloo and the Region of York.
- Communicate with external and internal stakeholders on all matters pertaining to quality.
- Promotes quality improvement achievements and identifies process improvement to increase the efficiency, effectiveness and quality throughout the organization.
- Establishes a continuous performance and quality improvement monitoring and reporting system. Regularly reports the status of performance and quality improvement efforts and impacts.
- Supports the development and implementation of quality assurance measurements and audits, and assists in improving processes throughout the organization through coordinating and engaging in activities to proactively promote implementation of evidence based best practices and resolve deficiencies.
- Conducts audits to ensure compliance with policies and procedures and accreditation requirements. Assures that quality assurance audits are completed in a timely fashion and evaluates/implements appropriate corrective actions.
- Identifies through the root analysis process a summary of issues that have the potential to negatively impact organization outcomes and/or the delivery of quality healthcare and provides solutions.
- Monitors and reports out on cases, occurrences, QRS, ETMS, and complaints to identify trends and emerging issues.
- Assist in the planning, coordination, facilitation and evaluation of QI learning sessions
- Responsible for conducting, receiving and analyzing Client Satisfaction Surveys for the Home Support, Nursing, and Rehab department.
- Works closely with management in each department to collect data, implement new protocols, and ensure each department is meeting set requirement/expectations.
- Prepare reports by collecting, analyzing and summarizing information and trends.
- Facilitate monthly CQI meetings.
- Assures that internal and external KPI’s targets are met, and if not identifies where improvement is required.
- Responsible for the completion of Client Chart Audits, analyzing results and provides strategies for improvement in quality of care.
- Complete assigned tasks as required.

**What you will need to succeed**:

- Post-secondary education, preference in Business or Math
- Minimum of 5 years’ experience in Home Health Care setting
- Previous quality, safety or regulatory experience preferred
- Experience with database/spreadsheets is considered an asset

**Job Types**: Full-time, Permanent

**Salary**: $65,000.00-$75,000.00 per year
