**Position summary**:
Reporting to the Manager of the Healthy Families team, the Family Home Visitor works with families to build on their strengths, provide teaching and support with parenting skills, connecting them with resources in the community, and by promoting the healthy growth and development of children.

The Family Home Visitor works with high-risk families during the prenatal period and with those who have children from 0-6 years of age, in accordance with the Ministry standards and agency policies. This work is done collaboratively as part of a multidisciplinary team under the guidance of an assigned Public Health Nurse.

**Responsibilities**:

- Schedules and maintains appointments for home visits.
- Develops a trusting professional relationship with the family.
- Assists families to identify their goals and provide support for the families to accomplish them.
- Helps families to develop problem-solving skills to further promote and support their independence and resourcefulness.
- Plans materials for education and demonstration in the home and/or through a virtual platform.
- Provides information and support to parents on topics such as pregnancy, child development, providing a safe and nurturing environment.
- Completes accurate and ongoing records of contact with families, meetings with other agencies and case conferences within program timeframes.
- Works collaboratively with the Public Health Nurse. Consults with the Family Home Visitor Lead and the manager as needed.
- Is able to work within a multi-disciplinary team environment.
- Provides regular updates to the assigned Public Health Nurse regarding family progress.
- Updates and/or consults with the Family Home Visitor Lead when there are concerns regarding a family file.
- Acts with professionalism and integrity, demonstrates and adheres to policies, procedures, ethics, and values of Public Health Sudbury & Districts.
- Maintains confidentiality in all issues relating to family care.
- Attend staff meetings regularly and participates in training opportunities when possible.
- Contributes to the development of program plans and develops, plans, implements, coordinates, and evaluates assigned projects in consultation with the Family Home Visitor Lead and other team members as required.
- Participates in the orientation of staff, students, volunteers, and others as may be required for the position.

**Requirements**:

- Minimum grade 12 diploma required. Post-secondary education in social work or child/youth worker programs or equivalent will be considered an asset.
- Demonstrated experience and genuine interest in working with and assisting high-risk families with young children who are facing challenging life situations is essential.
- Familiarity with the unique needs of the community to be served, with consideration towards language, culture, ethnicity, age group and socioeconomic status.
- Ability to interact effectively and respectfully with diverse individuals, groups, and communities.
- A good listener, who demonstrates compassion, ability to nurture, flexibility, patience, and ability to tolerate rejection.
- Demonstrated ability to effectively problem-solve and the ability to support and empower others to solve their own problems.
- Extensive knowledge and the ability to navigate and link families to community resources and organizations.
- Ability to work both as a part of a multi-disciplinary team and independently.
- Excellent communication skills.
- Advanced oral and written proficiency in English is essential.
- Advanced oral and written proficiency in French is required for 1 position.
- Strong computer skills and an ability to learn new computer programs is essential.
- Must be prompt, reliable and maintain a good attendance record.
- Maintains current Ontario Driver’s license with agency required insurance and has access to a reliable vehicle to fulfill position requirements. Travel within the district of Sudbury is a requirement of this position.
