This position oversees all aspects of client care to ensure that the quality of care is consistent with the mission and vision of Memories+ Home Healthcare’s high standards of care.

**Key Activities and Responsibilities**:
**_Client care management: _**
- Performs initial and ongoing client assessments.
- Identifies and responds to issues affecting client safety;
- Calls / visits clients on a regular basis during or outside hours of care, to monitor client status and satisfaction.
- Monitors each client’s well-being and observes for signs of ill health, ensuring that appropriate professionals are involved in the case at all times.
- Makes recommendations for changes to the client care provided if needed, and informs the Powers of Attorney, when necessary, of any changes in the client’s case and condition / communicates with appropriate health care professionals as required.
- Investigates all client complaints and reports all actions in the clients’ files.
- Provides advice in emergency medical situations as required.
- Updates ClearCare each time a call is taken or field visit made.

**_Field staff management: _**
- Supervise field staff and provide regular feedback / coaching to ensure Memories+ Home Healthcare quality standards and protocols are met at all times.
- Complete Field Staff performance evaluations (probationary, annual and disciplinary) as required.
- Promote staff development plans and provide opportunities for continued education of staff.
- Prepares, facilitates and leads Orientation and In-Service training sessions, staff meetings for employees on an ongoing basis as required.
- Works alongside Administrator and/or Executive Director to create master schedule on Monthly basis based on staff availability and any changes on a regular basis, notifying staff and clients of changes. Call employees to offer and confirm shifts as required.

**_Business development and community outreach: _**
- Develop and nurture positive professional relationships with key community partners as required for client’s care coordination and business development.
- Coordinate and facilitate community info sessions on a regular basis.
- Supports Health and wellness clinics, trade show tables and information sessions in community as required.
- Works closely with Executive Director on special assignments relating to training, Community and Employee Relations and other specific Memories+ Home Healthcare programs.

**_Other: _**
- Perform on-call Phone coverage shifts as required.
- Answer phones and screen enquiry calls with intent to set up assessments on an as-required basis.

**Job Types**: Full-time, Permanent

**Salary**: $72,000.00-$82,000.00 per year
