Social workers in health care provide in-depth bio-psychosocial assessments, diagnostic, treatment and evaluation services for individual, family, group and couples. This position involves counseling, discharge planning, resource referral, consultation and collaboration, teaching, research and patient/family education.

The outreach social worker with the St. Michael’s Hospital Academic Family Health Team will be an asset to facilitate connection between community members and our interprofessional primary care team. The outreach social worker works collaboratively with community partners in the downtown east of Toronto, including Moss Park, St. Jamestown, and Regent Park, to connect with community members who face significant financial challenges, housing insecurity, and other forms of marginalization.

The outreach social worker will provide direct psychosocial care to new primary care patients in collaboration with the interprofessional team through our new patient pathway. This social worker provides care to patients onsite at our primary care clinics, in patient’s homes and in the community. Individuals with lived experience of being marginalized by poverty and discrimination are strongly encouraged to apply.

**DUTIES & RESPONSIBILITIES**:

- Able to work effectively as a member of an inter-professional team, collaborating with other disciplines to provide comprehensive patient care
- Actively contributes to the academic mandate of the organization as an educator, researcher, resource or support for staff, students and members of the academic and professional community
- Travel in the community is required
- Identifies and addresses educational needs for patients and their families
- Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care
- Is committed to, and participates in, ongoing quality improvement in the organization
- Practices from a trauma informed, anti-oppressive and anti-racist perspective.
- Provides patient care in accordance with all professional and organizational standards
- Demonstrates understanding of the social determinants of and knowledge of how to access and link patients to appropriate community resources and services such as: OW, ODSP, Trillium, Community Housing, etc.

**QUALIFICATIONS**:

- Must have a Master's Degree in Social Work (MSW) or Equivalent
- Eligible to engage in student education with academic partners.
- Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas.
- Individuals with lived experience of social marginalization by poverty and discrimination are encouraged to apply
- Current Basic Life Support (BLS) certification (available on site)
- Excellent communication, critical thinking and interpersonal skills
- Proven flexibility and adaptability in a fast-paced environment
- Excellent time management, organizational and prioritization skills as relates to patient care.
- Must be willing to work evenings and/or weekends as directed by the Program.
