**Location**: Faith Place, 44 William St., Oshawa, ON
**Position Type**: Permanent, Full-Time (30 hrs/week, Monday to Thursday)
**Vacation**: Two weeks paid vacation
**On-Call**: Every other week (1 hour pay per day on-call)

**Position Overview**

Faith Place Support Services is seeking a dedicated and experienced professional to oversee the Supportive Housing Program, providing leadership and direction to both clients and Personal Support Workers (PSWs). The responsibilities of the **Support Services Manager**, ensures continuity of care and organizational excellence within a dynamic, client-centered environment.

**Key Responsibilities**

**Program Management & Leadership**
- Provide overall leadership and management of the Supportive Housing Program
- Collaborate with the Administrator and PSW Team Leader to maintain high standards of care
- Represent and advocate for clients in areas of personal support and care needs
- Develop, implement, and update policies and procedures for the program
- Supervise and provide support to PSW staff, including recruitment, training, scheduling, and performance evaluations
- Ensure compliance with health and safety protocols, including incident and WSIB reporting
- Monitor staff attendance, timekeeping, and documentation accuracy

**Client Care Coordination**
- Conduct client assessments, reassessments, and develop individual service plans
- Maintain accurate client documentation including rights, responsibilities, consent, and emergency contact information
- Install and provide orientation for Lifeline monitoring systems
- Liaise with Ontario Health at Home case managers and make referrals for professional services as needed

**Administrative Duties**
- Communication between Community Care Durham regarding the Support Program for Faith Place, including bi-monthly Zoom meetings
- Maintain efficient operations of the Supportive Housing Office
- Handle documentation related to payroll, benefits, and tax compliance in coordination with the bookkeeper
- Work with Team Lead to Track and manage inventory for supplies (gloves, cleaning, Lifeline, etc.)
- Contribute to budget planning and control in partnership with the Administrator
- Oversee scheduling for client care services to assist Team Lead with implementing personal care, housekeeping, and laundry
- Maintain up-to-date staff files and PSW certification records

**Team Supervision & Support**
- Promote teamwork and positive communication among PSW staff
- Act as a resource for staff, clients, and families to address routine care or work-related issues
- Carry out orientation and training for new PSWs
- Sign off on timesheets and replace staff as needed due to absences
- Cover frontline PSW duties when required and provide after-hours on-call support (alternating weeks)

**Additional Support**
- Lead Chair Exercises in the building twice a week (Tuesdays and Thursdays)
- Plan and program Annual Strawberry Social (June/July)
- Advocate for Residents in the building requiring extra help or care

**Qualifications**

**Education**
- Post-secondary diploma or degree in Gerontology or a related Community Health field
- Personal Support Worker Certification; additional education in Health or Gerontology an asset

**Experience**
- 2-3 years of leadership experience in a community setting supporting elderly and/or physically disabled clients
- 3-5 years of hands-on experience as a PSW in a community setting

**Terms of Employment**
- Regular work schedule: Monday-Thursday (30 hours/week)
- On-call every other week (remuneration provided)
- 2 weeks paid vacation annually
- Required to cover shifts if no alternate staff are available
- Not regularly required to work statutory holidays
