**Key Responsibilities**:

- Adheres to the personalized plan of care and assists in the delivery of personal care and activities of daily living, including assistance with ambulation, positioning and transferring that may require the use of a mechanical/ Hoyer lift.
- Assisting or providing total personal care related to toileting, bathing and perineal care, eating dressing and personal hygiene where indicated.
- Facilitate or support with Residents’ move in and transfers where applicable.
- Observe and report changes in Residents/ condition/behaviors to the nursing supervisor; to ensure appropriate follow up and assessment.
- Immediately communicate medical emergencies to the nursing supervisor or contact emergency medical services as appropriate or as directed.
- Demonstrate an understanding of Resident abilities/ required supports while fostering and encouraging independence where Residents are capable to maximize self-sufficiency.
- Provide Residents with cognitive, social, emotional, and psychological support.
- Maintain Resident dignity, privacy and confidentiality.
- Respond to Resident//family concerns immediately by reporting to management to ensure appropriate follow up or corrective action where required.
- Assist in maintaining a safe and clean environment, ensuring care equipment and assistive devices are in working order prior to use, Resident environment/including belongings, are decluttered and secured as required.
- Observe & report concerns to nursing supervisor related to Resident safety to implement interventions as required to mitigate risk.
- Engage/collaborate with other agencies that may be involved in Resident care as directed.
- Attend in services/ education when offered/as required to develop, reinforce or enhance knowledge and skill.
- Uphold/promote organizational values and philosophy’s relating particularly to ethics, morality, and integrity as set out in the Code of Conduct.
- Complete all other tasks as assigned.
