**Position Description**

**Position: Case Management Worker**

**Position Profile**: The Case Manager will work to support individuals who have been involved in a high-risk lifestyle, with the philosophy of building on strengths and supporting challenges. This will require providing one on one support as well as working with various professionals and support agencies to coordinate meeting the daily living, health, financial and social needs of individuals and/or families who have been involved in high-risk lifestyle.

**Position Reporting**: This position reports to the Executive Director/Directors

**Major Duties and Responsibilities**:
▪ Support families with a philosophy of building on strengths, supporting challenges, and promoting community inclusion and interdependence

▪ Conduct the move in/move out report with client, including preparing suite for next client

(laundry)

▪ Work through the intake process with clients, opening file, collecting information and determining areas of strength and challenges to support with clients and their families

▪ Carry out assessments of clients, as well as other assessments on an ongoing basis to determine needs

▪ Case Management, Crisis Intervention procedures, child protection, and child-in-care services and protocols

▪ Foster a team approach with other staff and stakeholders

▪ Provide one on one support to clients to create stability and a personally satisfying quality of life

▪ Developing and implementing support plans with solution focused and strength-based theories

▪ Provide one-on-one support to clients in: o Accessing required services and resources, including income security, health care, etc. o Assisting in money management and bill paying
- Building on strengths and interests to become involved in recreation and develop pro social relationships o Assisting in decision making, as well the many tasks that are part of daily living o Educating clients on effective parenting techniques o Setting healthy boundaries o Nutrition and meal preparation o Establishing routines o Assist with childcare when required

▪ Assist in the development of and access to resources in the community

▪ Keep contact notes on a regular basis

▪ Provide travel when required for programming and doctor appointments

▪ Oversee and witness clients taking their medication(s)

▪ Other duties as assigned by Director(s)

**JOB RELATED COMPETENCIES**

▪ Bachelor’s degree in Social Work or related field of study and/or a suitable combination of training and extensive experience in working with individuals who have been involved in a high-risk lifestyle.

▪ Ability to speak a First Nations or Métis language is an asset.

▪ Knowledge and understanding of the history of Aboriginal peoples and their socio
- economic backgrounds.

▪ An understanding of the barriers to healthy living that can contribute to higher risk for child abuse and neglect.

▪ An understanding of child development and age-appropriate developmental needs of children.

▪ Must possess and understanding of positive and culturally appropriate approaches to parenting.

▪ Must be knowledgeable of the dynamics of living with HIV/AIDS.

▪ Must be knowledgeable with Fetal Alcohol Spectrum Disorder and other cognitive disabilities.

▪ Must have knowledge of both solutions focused and strength-based theories.
