**Job Description**:
In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Integrated Service Coordinator, the Community Primary Care Team Lead is responsible for leading an integrated interdisciplinary team of community based health care professionals and support staff; determines workload assignments; schedules and coordinates the efficient use of staff, equipment and other resources to ensure delivery of appropriate and safe client care. The Community Primary Care Team Lead plans, organizes, coordinates, participates in and evaluates the delivery of integrated health care services. This position assists with program development and continuous quality improvement for the integrated community primary care team and OHC service model implementation to ensure referred clients receive optimal care and service integration.

**QUALIFICATIONS**:
**Education, Training And Experience**:
Graduation from a recognized nursing program plus three (3) years' recent, related experience, including one (1) year relevant clinical and two (2) years supervisory experience or an equivalent combination of education, training and experience.

Recognized post basic education related to gerontology, chronic care and care of the older adult; knowledge of and experience working with related legislation; or an equivalent combination of education and training. Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant. Valid BC Drivers' License
