**Prison In Reach Community Development Coordinator**

**DESCRIPTION**
This program aims to increase access to information and resources for people incarcerated in federal prisons living with and at risk of HIV/HEP C infection. In addition, program staff provide individual support and advocacy to increase linkage to testing, treatment and support for prisoners/ex-prisoners living with and at risk of Hep C. The coordinator will work with community partners to create accessible, culturally safe pathways to healthcare and social services.
**Job Duties**:
- Provide case management support and advocacy for prisoners/ex-prisoners living with and at risk for acquiring HIV and Hep C
- Provide reintegration, system navigation support for prisoners/ex-prisoners living with and at risk of acquiring HIV/Hep C prior to and upon their release for prison.
- Foster partnerships and collaborate with diverse community stakeholders to dismantle systemic barriers for people during the reintegration process and increase access to HIV, HEP C testing and treatment.
- Develop relevant HIV, Hep C and Harm reduction educational resource materials with federally incarcerated people
- Support the development of stigma free, culturally safe services for prisoners/ex-prisoners through the delivery of capacity building service provider training.
- Develop and maintain relationship with relevant correctional staff to coordinate and deliver Hep C, HIV and Harm Reduction in-reach programming
- Develop and implement workplans and evaluation tools to measure program impact
- Participate in regular supervision and team meetings, and agency events.
- Maintain records, reports, and statistics necessary for program functioning, monitoring, evaluation, and reporting.
- Attend program, department and agency meetings and trainings.
- Perform other duties as required
**Qualifications**:
- Combination of some post-secondary education and significant experience in health promotion and/or community development
- Three to five years relevant harm reduction experience in a community setting working with homeless and marginalized populations including prisoners/ex-prisoners, people with mental health issues and people who use drugs.
- Understanding of the health and social impact of the criminal justice system on individuals and society and experience working with HIV and HEP C issues including a well-developed knowledge of relevant resources
- Experience in working with people vulnerable to incarceration, prisoner/ex-prisoner communities.
- Understanding of Hep C, drug use and harm reduction in prison context, especially in communities who are disproportionately criminalized.
- Strong knowledge of harm reduction and opioid overdose prevention strategies.
- Experience developing, facilitating, and evaluating targeted education and training workshop.
- Expertise in community development focusing on marginalized communities.
- Experience working within an anti-racist / anti-oppressive framework
- Demonstrated skills preparing presentations, reports, program plans, educational materials, brochures, etc.
- Ability to develop and sustain partnerships, coordinate networks and support advocacy strategies
- Demonstrated experience with case management
- Demonstrated ability to work under pressure meet deadlines and ability to work both independently and within a team structure.
- Demonstrated ability to anticipate problems/conflicts and take appropriate actions, to organize and prioritize work.
- Sound decision-making and judgment skills with the ability to effectively manage communications with community, committees, and coalitions.
- Demonstrates a commitment to a healthy and safe workplace for self and others by taking all reasonable precautions and working in compliance with PASAN’s related policies, health and safety legislation and best practice.
**Position Requirements**:
