**Primary Responsibilities**:

- Leads a team of Personal Support Workers, contracted staff and students in the delivery of in-home care to seniors across the spectrum of MAPLe scores.
- In collaboration with Care Navigators, participates in developing and assessing service plans, and in ensuring that PSWs deliver the highest calibre of in-home health services within the PSW scope of practice. The Supervisor, In Home Services ensures compliance to the LHIN and Accreditation Canada Home and Community Care standards, and to the NWT OHT requirements.
- Embraces the standards outlined in the Best Practice Standards Organization (BPSO) guidelines outlined by the RNAO and translates those standards into measurable outcomes and behaviours for PSWs.
- Oversees the day-to-day operations of in-home services, including client admissions, discharges; as well as staff scheduling and management. Ensures high quality documentation and client record keeping.
- Assists in the implementation processes to support best practices as well as the delivery of safe, effective, efficient and quality services.
- Collaborates with partners, represents agency externally at planning meetings.
- Support the ongoing development of a culture of safety, excellence in quality care and risk management by implementing the integration of a strategy framework of continuous quality improvement.
- On call duties required.

**What We’re Looking For**:

- Allied Health Professional (Registered Practical Nurse and/or Social Service Worker Diploma) is preferred. Diploma in a relevant discipline within the associated fields required.
- Minimum of two years’ experience within the community support service sector or health care environment, working with a senior population.
- Minimum of two years’ supervisory experience of unregulated staff preferred.
- Knowledge of the Inter-RAI CHA assessment tool and of CIMS is an asset.
- Excellent understanding of the aging process, dementia and the specific needs of the elderly, disabled and caregivers required.
- Understand in-home health care issues, and familiarity with the Assisted Living Services for High-Risk Seniors Policy.
- Have excellent computer skills with proficiency in MS Word, Outlook and Power Point.
- Have a Valid Ontario Driver’s License and access to a reliable vehicle
- Valid Vulnerable Police Record check completed within last 12 months.
- Current CPR/First Aid Certification.
- Fluency in other language(s) is an asset.
- Mandatory COVID 19 vaccination is required

**Job Types**: Full-time, Permanent

**Salary**: $48,000.00 per year
