**Position:**:Personal Support Worker - Home Support and Day Program (Part-Time)
**Hours of Work:**:Variable based on client demand
**Reports to:**:Home Support Supervisor
**Deadline**: Until position is filled
**Position Summary**:
Responsible to provide personal support services to enable client seniors to continue living with independence, and to be healthy and safe-at-home. Ensures satisfaction with the services by establishing and maintaining relationships with client seniors and their care providers and/or family members.
**Responsibilities**:
**1. Provides personal support services to community Clients, as directed by the service and/or care plan and following procedures in the Home Support Services Handbook**:
- Provides personal care related activities of daily living: bathing, washroom assistance, changing incontinence products, medications, dressing, personal hygiene and grooming and dental care
- Assists with transferring and positioning of Client
- Assists with meal preparation, set-up and eating
- Obtains approval from supervisor for any changes to the work schedule
- Promotes an inclusive environment for Clients respecting abilities, age, ethnicities, languages, genders, sexual orientations
**2. Provides Home Making services to community Clients**:
- Provides light housekeeping services to community Clients
- Completes household tasks according to the Home Making Components and the care plan
**3. Provides personal support services to Clients and daily activities in the Day Program, per the service and/or care plan and the procedures in the Day Program Handbook**:
- Provides personal care related activities including but not limited to: feeding, cleaning-up, toileting and hygiene, medication reminders
- Assists with cleaning and personal hygiene after Client incontinence
- Assists for program activities and daily routines, including cleaning, sanitizing ensuring hygiene of program areas at all times
- Assists Day Program staff including volunteers and placement students
- Attends and participates in case conferences and other organization and department activities when needed
**4. Documents services and reports concerns and occurrences**:
- Reports to the Care Manager and/or Supervisor about any unusual occurrences such as health concerns or emergencies, Client requests for different services, etc.
- Reports to Day Program Team Leader about changes in Day Programs Clients’ social, physical and mental status
- Clock in/out and electronically documents care plan tasks with the AlayaCare Mobile app following every visit
**5. Promotes health and safety of Clients and Self**:
- Works safely at all times and ensures the safety of Self, Co-workers and Clients by preventing hazards and report to the Supervisor
- Complies with all organizational and departmental Health and Safety policies and procedures.
Any other reasonable duties that may be assigned from time to time
**Education**:
- Personal Support Worker Certificate
- Valid First Aid and CPR
**Skills and Experience**:
- Experience working with seniors in a community program setting
- Good communication and interpersonal skills
- Ability to write clear and concise client notes in English
- Ability to physically assist seniors with sitting, standing, bending, bathing, etc.
- Basic understanding of social, recreational or therapeutic programs an asset
- Ability to maintain confidentiality and the knowledge and sensitivity to work in a culturally diverse environment
- Knowledge of health care, dietary needs or limitations of individuals with special needs as a result of ageing e.g. Alzheimer's, stroke, Parkinson's disease
- Ability to physically assist clients with program activities, eating, toileting, personal and hygiene care, mobility
- Sound judgment, initiative and the ability to work in a self-directed manner.
- Physician’s medical assessment required (A TB test form will be provided)
- Must be available to work on evening and overnight shifts
- Vulnerable Sector screening required
- Proof of full COVID-19 Vaccination or Medical Exemption Certificate required
**Working Conditions**:
- Standing and walking for long periods of time and frequent bending
- Exposure to agitated and confused Clients
- May be exposed to contagious diseases
- Required to travel to Clients’ homes; reliable transportation required for Home Support Services
- Working alone in Clients’ homes for Home Support Services
