Central Toronto Youth Services (CTYS) is a community-based Children’s Mental Health Centre committed to responding to the needs of young people, their families, and the youth-serving community. We provide innovative approaches to the care of youth through advocacy, system navigation, and direct-service programming through case-management and counselling.

**Early Intervention in Psychosis Program (EI) within New Outlook at CTYS**

The **Early Intervention Program** is one of 5 New Outlook Programs funded by Ontario Health. The Early Intervention Program (EI) is a community-based counselling and intense case management program that provides up to 3 years of service to young people (14-24) who are experiencing psychosis or exhibiting prodromal symptoms. As a member of the EI team, you will work with clients and their significant others to improve their quality of life and instill hope.

The Service Coordinator role will focus on functioning as a clinician case manager within an interprofessional practice team. In this role, you will be expected to assess, plan, implement and evaluate clinical care utilizing a newly adopted care pathway that is designed to deliver standardized, evidence-based treatments including the delivery of the NAVIGATE Early Intervention Model of Care; in this role you will provide the **Individual Resiliency Training** (IRT) component of this intervention model. This model provides a framework for mental health assessment, monitors client responses to pharmaceutical and care interventions, with an emphasis on resilience and collaborative care. This position requires a sound knowledge of psycho-social rehabilitation, crisis intervention, addiction issues, severe and persistent mental illness, and an understanding of schizophrenia, primary mood disorders with psychosis, and other presentations of psychosis.

Through the exploration of client experiences of mental health, staff will build relationships and engage both clients and their significant others to access resources and opportunities. EI staff also integrate interventions that are trauma informed, and evidence based and take into consideration ways to prioritize social justice and inclusion.

**Primary Responsibilities**:

- To provide clinical services, informed by Provincial Early Psychosis Intervention Program Standards
- Utilizing NAVIGATE Early Intervention Model of Care, work collaboratively with youth, and other service providers, as well as parents/caregivers to develop comprehensive and effective service delivery plans for transitional aged youth 14-24 experiencing psychosis or exhibiting prodromal symptoms
- Learn and integrate NAVIGATE Model of Care - attend consultations and trainings as directed
- Provide intensive case management supports
- Work collaboratively with other disciplines and roles as outlined in the NAVIGATE Model of Care
- Provide client support and psycho-education specific to psychosis and recovery
- Provide a combination of virtual and in-person services in the office and/or community
- Provide counselling and therapeutic services, informed by trauma theory, and evidence informed practices including CBT, MI, and recovery frameworks
- Within above approaches, integrate knowledge of anti-oppression/anti-racism frameworks
- Assist youth in identifying and acquiring the skills needed to meet their goals
- Support advocacy and resource brokerage
- Facilitate access to resources such as psychiatry, housing, substance abuse programs, crisis response services etc. and to provide crisis response/safety planning as necessary
- As required, work with families from diverse cultures, utilizing interpreters as needed
- To fulfill the above mentioned responsibilities in a flexible manner which may include some evening work
- Participate in team, staff, and supervision meetings
- Maintain all records and reports as required by the CTYS policies and procedures

**Qualifications**
- BIPOC Identification, member of Black/A.C community prioritized
- MSW or equivalent education and experience
- Membership in Regulated Health College (R.P./RSW)
- Demonstrated experience working from an anti-oppressive framework with youth and families from diverse ethno-cultural and socio-economic backgrounds including youth with diverse intellectual abilities and racialized communities
- Minimum of 3 years’ experience in a mental health setting
