**About the Role**:
In alignment with the **Mission, Vision, Values**, and strategic directions of Providence Health Care, the Social Worker promotes a safe, respectful, and inclusive environment for patients, families, and staff. You will work collaboratively within an interdisciplinary team under the direction of the **Patient Care Manager**, with clinical guidance from the **Social Work Site Leader**, **Professional Practice Leader**, and **Research, Education and Practice Coordinator**.

**Key Responsibilities**:
- Deliver **therapeutic clinical counseling** to individuals with complex substance use and mental health needs.- Complete **comprehensive biopsychosocial assessments** to inform treatment planning.- Act as a **leader in discharge planning**, collaborating with care teams, families, and community partners.- Offer **consultative services** on community resources and access pathways for clients and families.- Coordinate and lead **clinical/case conferences** and **in-service education sessions** for staff.- Actively participate in **committees, working groups**, and quality improvement initiatives.- Maintain **up-to-date clinical documentation** and stay informed on professional developments and best practices in Social Work.

What You Bring:
**Qualifications**:

- ** Master’s in Social Work** from an accredited university
- ** One year of recent related experience** in a designated program area, or an equivalent combination of education, training, and experience
- ** Current full registration** with the British Columbia College of Social Workers
- Bachelor of Social Work degree from an accredited School of Social Work, with an equivalent combination of education, training and experience may be considered. Current full registration with the British Columbia College of Social Workers

**Skills and Abilities**
- ** Demonstrated ability to communicate** effectively with patients/residents, families, staff, physicians, and other healthcare providers
- ** Knowledge of legislation and government policies** related to healthcare
- ** Ability to gather relevant data** and conduct biopsychosocial assessments
- ** Experience in conducting patient/resident assessments**:

- ** Knowledge of other healthcare disciplines** and their roles in patient/resident care
- ** Ability to draw out information** to understand the patient's/resident's situation and explore issues beyond initial concerns
- ** Proven ability to work collaboratively** as a member of an interdisciplinary team
- ** Skill in conflict resolution**, including guiding others through resolution issues
- ** Physical ability** to perform the duties of the position
- ** Strong independent work skills**, with the ability to prioritize tasks and workload
- ** Proficient in operating related equipment**:

- ** Willingness and ability to participate in research**:

- ** Experience in providing supervision and teaching** to students
- ** Ability to facilitate and influence group and inter-group meetings**, processes, and decision-making
