**Job Summary**

**R**esponsibilities:

- Conduct thorough patient assessments, including medical history and physical examinations.
- Diagnose and treat acute and chronic illnesses, ensuring appropriate medication administration.
- Develop individualized treatment plans based on patient needs and preferences.
- Provide preventive care services, including immunizations and health screenings.
- Utilize MedAccess or similar electronic health record systems for data collection and documentation.
- Collaborate with specialists for referrals and coordinate care for complex cases.
- Educate patients and their families about health management, medication use, and lifestyle changes.
- Participate in research initiatives to enhance clinical practices and patient outcomes.
- Maintain up-to-date knowledge of medical terminology, procedures, and advancements in family medicine.

**Skills**
- Strong patient care skills with a focus on empathy and effective communication.
- Experience in hospital settings as well as home care environments is preferred.
- Proficiency in using electronic health record systems such as MedAccess for efficient documentation.
- Ability to perform data collection accurately for ongoing patient assessments.
- Knowledge of medication administration protocols and acute care practices.
- Familiarity with pediatrics is a plus, enabling comprehensive family-focused healthcare delivery.
- Strong analytical skills for research purposes and improving clinical practices.
