**Summary**:
Within the context of a client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, the person promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.

Reporting to the Program Director, this key role provides operational leadership and support to interdisciplinary team members who are part of the Provincial COVID Recovery Network providing care to Post-COVID patients who experienced serious disease and need support to manage their condition over time. This position maintains responsibility in collaboration with Physician Leaders for the effective and efficient delivery of services within the Provincial COVID Recovery Network. The role promotes and facilitates the continuum of care delivery through interdisciplinary clinics that are integrated with primary care providers and based on a model of supportive care co-designed by clinicians and survivors of COVID-19. This position works collaboratively with provincial partners, Program Leaders, other Managers, Practice Leaders, and Corporate and Support Leaders, building and enabling partnerships and clinical care coordination to provide provincial clinics with best practices, evidence and leadership in this interdisciplinary model of care.

**Qualifications / Skills and Education**:
Education
Master’s degree in a related Health Profession plus a minimum of seven (7) years’ experience at the middle management level in a medium to large health care organization or an equivalent combination of education and experience.

**Duties and Responsibilities**:
Provides operational leadership to an interdisciplinary team to ensure patient/resident/family focused care is delivered within a coordinated and integrated care delivery model.

Collaborates internally with the Physician Operations Leader and externally with other members in the COVID Recovery Network (“Network”) to plan and manage the provision of effective and efficient care within the Post-COVID Recovery Clinic to support the achievement of operational and strategic objectives of the clinical program and consistent with the mission, vision and values of PHC.

Works with community and/or internal partners as appropriate to ensure the provision of timely care to meet the needs of patients/residents/families. Assists teams within the Network to manage risks, incidents, family/client complaints and discharge planning.

Ensures effective, two-way communications with and among team members so that each team is aware of and understands changes impacting them, the site, the organization and how such changes will impact delivery of services.
